经皮椎体成形术与经皮球囊扩张椎体成形术治疗骨质疏松性椎体压缩性骨折的临床疗效比较

摘　　要：目的探讨骨质疏松性椎体压缩性骨折采用经皮椎体成形术与经皮球囊扩张椎体成形术治疗的临床疗效差异。方法将南平市建阳第一医院2020年1月至2023年2月收治的78例骨质疏松性椎体压缩性骨折按治疗方法不同分为对照组(n=39)和观察组(n=39),分别采取经皮椎体成形术(对照组)和经皮球囊扩张椎体成形术(观察组)治疗,比较两组患者手术指标包括手术时间、总失血量、骨水泥用量、骨水泥渗漏率、术前及术后2周VAS评分、伤椎后凸角度及椎体前缘高度,术前及术后3个月ODI评分和SF-36评分。结果观察组手术时间长于对照组,骨水泥用量多于对照组,总失血量和骨水泥渗漏率均少于对照组(均P<0.05)。术后2周,观察组腰背疼痛VAS评分、伤椎后凸角度均低于对照组,椎体前缘高度高于对照组(均P<0.05)。术后3个月,观察组ODI评分低于对照组,SF-36评分高于对照组(均P<0.05)。结论骨质疏松性椎体压缩性骨折采用经皮椎体成形术与经皮球囊扩张椎体成形术治疗各有优劣势。Objective To investigate the clinical effect of percutaneous vertebroplasty and percutaneous balloon dilated vertebroplasty for osteoporotic vertebral compression fracture and its influence on quality of life.Methods A total of 78 cases of osteoporotic vertebral compression fractures admitted to Jianyang First Hospital of Nanping City from January 2020 to February 2023 were divided into control group(n=39)and observation group(n=39)according to different treatment methods.Percutaneous vertebroplasty and percutaneous balloon dilated vertebroplasty were performed,respectively.Clinical efficacy were compared between the two groups.Results Compared to the control group,the observation group had longer operation time,more amount of bone cement,and less blood loss and cement leakage rate(all P<0.05).At 2 weeks after surgery,compared to the control group,observation group had lower VAS score and angle of posterior convexity of the injured vertebra,and higher height of anterior margin of vertebral body(both P<0.05).At 3 months after surgery,compared to the control group,observation group had lower ODI score and higher SF-36 score(both P<0.05).Conclusions There are advantages and disadvantages of percutaneous vertebroplasty and percutaneous balloon dilated vertebroplasty in treatment of osteoporotic vertebral compression fractures.
