摘　　要：目的 探讨同期行鼻内镜下鼻中隔矫正术联合鼻骨复位对鼻骨骨折患者鼻腔结构及鼻功能的影响。方法 选择2020年6月至2022年6月收治的鼻骨骨折患者88例,随机抽签法分成复位组和手术组,每组44例。复位组开展鼻骨复位术,手术组在复位组基础上行鼻内镜下鼻中隔矫正术。术后6个月评价疗效,评定鼻美观度、通气性、嗅觉功能等3项主观指标,测定鼻腔最小横截面积(NMCA)、鼻腔容积和鼻总呼气等3项客观指标,统计术后并发症发生率。结果 手术组治疗优良率为93.18%高于复位组的75.00%(P<0.05);术后6个月,2组鼻美观度、通气性、嗅觉功能评分均低于术前(P<0.05),且手术组3项主观指标均低于复位组(P<0.05);术后6个月,2组NMCA、鼻腔容积和鼻总呼气均大于术前(P<0.05),且手术组3项客观指标均大于复位组(P<0.05);手术组术后并发症发生率为6.82%,复位组为13.64%,差异无统计学意义(P>0.05)。结论 对鼻骨骨折患者实施鼻内镜下鼻中隔矫正术联合鼻骨复位术治疗,其疗效优于单用鼻骨复位术,可明显增加鼻部美观度,有效改善鼻腔通气功能及嗅觉功能。Objective To explore the impacts of nasal septum correction under nasal endoscope combined with simultaneous nasal bone reduction on nasal cavity structure and nasal function in patients with nasal bone fracture.Methods A total of 88 patients with nasal bone fracture admitted to our hospital from June 2020 to June 2022 were randomly grouped into reduction group(n=44)and simultaneous operation group(n=44).Patients in both groups were managed by nasal bone reduction,and those in the simultaneous operation group were additionally managed by nasal septum correction under nasal endoscope.Six months after the operation,the curative effect,three subjective indexes of nasal aesthetics,ventilation and olfactory function,three objective indexes of nasal minimum cross-sectional area(NMCA),nasal volume and total nasal exhalation,and the incidence of postoperative complications were compared between the two groups.Results The excellent and good rate of treatment in the simultaneous operation group was significantly higher than that of reduction group(93.18%vs 75.00%,P<0.05).Six months after operation,the scores of nasal aesthetics,ventilation and olfactory function in both groups were lower than those before operation(P<0.05),the three subjective indexes in the simultaneous operation group were significantly lower than those of the reduction group(P<0.05).Six months after operation,the NMCA,nasal volume and total nasal exhalation in the two groups were significantly higher than those before operation(P<0.05),and the three objective indexes in the simultaneous operation group were significantly higher than those of the reduction group(P<0.05).There was no significant difference in the incidence of postoperative complications between the simultaneous operation group and the reduction group(P>0.05).Conclusion Nasal septum correction under nasal endoscope combined with simultaneous nasal bone reduction is better than nasal bone reduction alone in the treatment of nasal bone fracture,which can obviously increase nasal aesthetics,a
