摘 要:目的探究宫颈脱落细胞miR-218-5p和叉头蛋白3(FOXP3)的表达与宫颈病变及人乳头状瘤病毒(HPV)感染状态的关系。方法选取2020年2月至2022年2月收治的98例慢性宫颈炎患者作为慢性宫颈炎患者组;宫颈上皮内瘤变(CIN)患者92例作为CIN组,其中CINⅠ级45例为CINⅠ组,CINⅡ级25例为CINⅡ组,CINⅢ级22例为CINⅢ组;宫颈鳞癌患者88例为宫颈鳞癌组。qRT-PCR法检测宫颈脱落细胞miR-218-5p表达;酶联免疫吸附测定法(ELISA)检测FOXP3以及炎性因子白细胞介素-2(IL-2)、白细胞介素-10(IL-10)、干扰素γ(IFN-γ)和肿瘤坏死因子(TNF)-α的水平;采用Pearson法分析miR-218-5p和FOXP3表达以及二者与炎性因子的相关性。Spearman法分析宫颈脱落细胞miR-218-5p、FOXP3表达水平与CIN组以及宫颈鳞癌组HPV感染的相关性。结果慢性宫颈炎患者组、CIN组、宫颈鳞癌组miR-218-5p水平依次显著降低,FOXP3水平依次显著升高,HPV感染率依次增加(P<0.05)。CINⅢ组miR-218-5p、IL-2的表达水平显著低于CINⅡ组及CINⅠ组,CINⅡ组显著低于CINⅠ组;CINⅢ组FOXP3、IL-10、TNF-α的表达水平显著高于CINⅡ组及CINⅠ组,CINⅡ组显著高于CINⅠ组,差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,miR-218-5p的表达水平与IL-2水平呈正相关(P<0.05),与FOXP3、IL-10、TNF-α水平呈负相关(P<0.05);FOXP3的表达水平与IL-2水平呈负相关(P<0.05),与IL-10、TNF-α水平呈正相关(P<0.05)。宫颈脱落细胞中miR-218-5p表达水平与CIN组以及宫颈鳞癌组患者HPV感染呈负相关(P<0.05),FOXP3表达水平与CIN组以及宫颈鳞癌组患者HPV感染呈正相关(P<0.05)。结论CIN以及宫颈鳞癌患者宫颈脱落细胞FOXP3的表达水平显著升高,miR-218-5p水平显著降低,二者与宫颈病变以及HPV感染状态有关。Objective To investigate the correlation of the expression levels of miR-218-5p and forkhead box protein P3(FOXP3)in cervical exfoliated cells with cervical lesions and human papillomavirus(HPV)infection status.Methods A total of 98 patients with chronic cervicitis admitted to Department of Obstetrics and Gynecology in our hospital from February 2020 to February 2022 were regarded as the chronic cervicitis group.During the same period,92 patients with cervical intraepithelial neoplasia(CIN)were classified as CIN group,including 45 CINⅠpatients,25 CINⅡpatients,and 22 CINⅢpatients;and 88 patients with cervical squamous cell carcinoma(CSCC)were included in the CSCC group.The expression of miR-218-5p in cervical exfoliated cells was detected by Quantitative real-time polymerase chain reaction(QRT-PCR).Enzyme linked immunosorbent assay(ELISA)was performed to detect the levels of FOXP3 and inflammatory factors like interleukin-2(IL-2),interleukin-10(IL-10),interferon-γ(IFN-γ)and tumor necrosis factor-α(TNF-α).Correlation of the expression levels of miR-218-5p and FOXP3 with inflammatory factors was identified by the Pearson correlation analysis.Correlation of the expression levels of miR-218-5p and FOXP3 in cervical exfoliated cells with HPV infection status in CIN group and CSCC group was identified by the Spearman’s correlation.Results The level of miR-218-5p in chronic cervicitis group,CIN group and CSCC group significantly decreased obviously,and the level of FOXP3 and the HPV infection rate significantly increased in sequential(P<0.05).The expression levels of miR-218-5p and IL-2 in CINⅢgroup were significantly lower than those of CINⅡand CINⅠgroup,and those in CINⅡgroup were significantly lower than those of CINⅠgroup.The expression levels of FOXP3,IL-10 and TNF-αin CINⅢgroup were significantly higher than those of CINⅡand CINⅠgroup,and those in CINⅡgroup were significantly higher than those of CINⅠgroup(P<0.05).Pearson correlation analysis showed that the expression level of miR-
