基于老年综合评估的干预对糖尿病合并老年综合征患者自我照顾能力的影响

摘　　要：目的研究基于老年综合评估的干预对老年糖尿病合并老年综合征患者自我照顾能力的影响。方法采取便利抽样的方法选取2019年10月至2021年5月广西医科大学第一附属医院老年病学内分泌代谢科收治的90例老年糖尿病合并老年综合征患者,根据电脑随机数字表法分为对照组和观察组,每组45例。对照组予以常规干预,观察组予以老年综合评估的干预,3个月后比较2组患者的代谢相关指标、自我照顾能力、自我管理能力及满意度。结果干预后,观察组患者的空腹血糖、餐后2 h血糖、糖化血红蛋白、收缩压、舒张压、总胆固醇、三酰甘油与低密度脂蛋白水平均低于对照组,而高密度脂蛋白水平高于对照组(P<0.05);观察组患者的自我护理能力测定量表(ESCA)评分高于对照组(P<0.05);观察组患者的糖尿病自我管理行为量表(SD-SCA)评分高于对照组(P<0.05);观察组患者的满意率高于对照组(P<0.05)。结论基于老年综合评估的干预在老年糖尿病合并老年综合征患者中的应用取得显著效果,不仅能改善其代谢相关指标,增强自我照顾能力与自我管理能力,而且还能促进其满意度提高。Objective To analyze the effect of intervention based on the comprehensive geriatric assessment on self-care ability of elderly diabetes mellitus(DM)patients with geriatric syndrome.Methods A total of 90 elderly DM patients with geriatric syndrome treated in the Department of Geriatrics,Endocrinology and Metabolism,the First Affiliated Hospital of Guangxi Medical University from October 2019 to May 2021 were selected by convenience sampling method.They were randomly divided into the control group and observation group according to computer random number table method,with 45 patients per group.Routine intervention,and the intervention based on the comprehensive geriatric assessment were applied to patients of control group and observation group,respectively.The metabolism-related indexes,self-care ability,self-management ability and satisfaction after 3 months of intervention were compared between the two groups.Results After intervention,patients in the observation group presented significantly lower fasting blood glucose(FBG),2h postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c),systolic blood pressure(SBP),diastolic blood pressure(DBP),total cholesterol(TC),triglyceride(TG)and low density lipoprotein(LDL),but significantly higher high density lipoprotein(HDL)than those of the control group(P<0.05).In the observation group,the Self-Care Ability Scale(ESCA)score,the Summary of Diabetes Self-Care Activites(SDSCA)score and the satisfaction rate were significantly higher than those of the control group(P<0.05).Conclusion The application of the intervention based on the comprehensive geriatric assessment in the elderly DM patients with geriatric syndrome can achieve significant outcomes,which improve their metabolic indicators,self-care and self-management ability,and their satisfaction.
