赛肤润联合凝胶体位垫在小儿脊柱侧弯矫正术不同部位压疮预防中的应用

摘　　要：目的探讨赛肤润联合凝胶体位垫在小儿脊柱侧弯矫正术不同部位压疮预防中的应用效果。方法选取2017年1月至2022年1月于河北省儿童医院行脊柱侧弯矫正术,年龄<12岁的患儿80例,男35例,女45例。采用随机数字表法将其分为4组(n=20):赛肤润组(S组)、凝胶体位垫组(G组)、联合组(L组)和对照组(C组)。患儿术中采用俯卧位,C组患儿额部、下颌部、胸部、髂前上棘、膝部及踝部放置棉垫,S组患儿在上述部位涂抹2滴赛肤润后按摩1 min,15 min后摆放体位。G组患儿在上述部位垫有凹槽的凝胶体位垫,L组患儿在上述部位涂抹2滴赛肤润后按摩1 min,15 min后摆放在有凹槽的凝胶体位垫上。观察患儿术后上述部位不同时间点压疮的发生情况及术后72 h疼痛情况。结果4组患儿一般资料统计差异无统计学意义(P<0.05)。与C组比较,S、G和L组患儿术后胸部、髂前上棘、膝部及踝部压疮的发生率降低,压疮程度及疼痛发生率减轻,差异有统计学意义(P<0.05)。与L组比较,S组患儿髂前上棘和踝部压疮的发生率增高,程度加重,差异有统计学意义(P<0.05),G组患儿胸部和髂前上棘压疮的发生率增高,程度加重,差异有统计学意义(P<0.05),2组患儿术后压迫部位疼痛发生率增加,其余部位比较差异无统计学意义(P>0.05)。与G组比较,S组患儿胸部压疮的发生率降低,程度减轻,踝部的压疮的发生率增高,程度加重,差异有统计学意义(P<0.05),其余部位比较差异无统计学意义(P>0.05)。结论赛肤润、凝胶体位垫及赛肤润联合凝胶体位垫均可有效减少小儿脊柱侧弯矫正术中压疮的发生概率及程度,赛肤润对于胸部压疮预防效果优于凝胶体位垫,凝胶体位垫对于踝部压疮预防效果优于赛肤润,赛肤润联合凝胶体位垫针对胸部、髂前上棘及踝部压疮的预防效果优于二者单独使用。Objective To explore the application of sanyrene combined with the use of gel position pad in the prevention of pressure sore at different parts in children treated with scoliosis surgery.Methods From January 2017 to January 2022,eighty children under 12 years old who were treated with scoliosis surgery in Hebei Children’s Hospital,including 35 males and 45 females were recruited.They were randomly divided into 4 groups(n=20 in each group):sanyrene group(group S),gel position pad group(group G),combined treatment group(group L)and control group(group C).The patients was placed in the prone position during surgery.In group C,cotton pads were placed on the forehead,mandible,chest,anterior superior iliac spine,knee and ankle.In group S,two drops of sanyrene were applied on the above parts and massaged for one minute.Fifteen minutes later,the positions were placed.In group G,grooved gel position pads were placed on the above parts.Children in group L were coated with 2 drops of sanyrene for 1 minutes,and placed on grooved gel position pad 15 minutes later.The incidence of erythema and pressure sore in the above parts and the pain at 72 hours postoperatively were recorded.Results There were no significant differences in the baseline characteristics among the four groups(P>0.05).Compared with group C,the incidence of pressure sores in the chest,anterior superior iliac spine,knee and ankle,the degree of pressure sores,and the incidence of pain were significantly lower in group S,G and L(P<0.05).Compared with group L,the incidence of pressure sores of anterior superior iliac spine and ankle,and the degree of pressure sores in group S were significantly higher(P<0.05).The incidence of pressure sores of chest and anterior superior iliac spine,and the degree of pressure sores in group G were significantly higher than those in group L(P<0.05).The incidence of postoperative pain in the compression site significantly increased in group S and G(P<0.05),and that in other parts was comparable(P>0.05).Compared with group G,the i
