针刺后溪穴联合桃仁杜仲汤加减治疗急性腰扭伤的临床疗效及对患者腰椎运动功能、生活质量的影响

摘　　要：目的 观察针刺后溪穴联合桃仁杜仲汤加减治疗急性腰扭伤的临床疗效及对患者腰椎运动功能、生活质量的影响。方法 将60例急性腰扭伤患者按照随机数字表法分为2组,对照组30例予常规西药治疗,治疗组30例在对照组治疗基础上予针刺后溪穴联合桃仁杜仲汤加减方治疗。2组均治疗10天。比较2组治疗前后中医证候评分、腰椎运动功能[Oswestry功能障碍指数量表(ODI)、Roland-Morris腰痛失能问卷(RMQ)]及生活质量[欧洲五维生活质量量表(EQ-5D)评分],并统计2组疗效。结果 治疗组总有效率90.00%(27/30),对照组总有效率73.33%(22/30),治疗组临床疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后ODI评分、RMQ评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后EQ-5D各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论 对急性腰扭伤患者采取针刺后溪穴联合桃仁杜仲汤加减治疗,临床疗效显著,可明显改善其临床症状,提升腰椎运动功能,改善其生活质量,值得推广应用。Objective To observe the clinical efficacy of acupuncture at Houxi(SI3)combined with modified Taoren Duzhong Decoction on acute lumbar sprain(ALS)and its influence on lumbar motor function and quality of life.Methods A total of 60 ALS patients were randomly assigned into the treatment group(n=30)and the control group(n=30).All patients were managed by conventional Western medicine,and those in the treatment group were additionally treated with acupuncture at Houxi(SI3)combined with modified Taoren Duzhong Decoction.Treated for 10 days,the aim was to compare traditional Chinese medicine(TCM)syndrome score,lumbar motor function(Oswestry Disability Index[ODI],Roland-Morris Disability Questionnaire[RMQ]),quality of life(European Quality of Life-5 Dimensions questionnaire[EQ-5D]).The curative effect was assessed.Results The overall effective rate in the treatment group was better than that in the control group(90.00%[27/30]vs 73.33%[22/30],[P<0.05],respectively).After treatment,TCM symptom scores in groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).The scores of ODI and RMQ in the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more prevalent compared with the control group(P<0.05).The scores of EQ-5D in the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more common than the control group(P<0.05).Conclusion For ALS patients,acupuncture at Houxi(SI3)combined with modified Taoren Duzhong Decoction has significant clinical efficacy,which can significantly improve clinical symptoms,lumbar motor function,and improve quality of life,and worthy of promotion.
