摘　　要：目的探讨髂腹股沟带蒂皮瓣修复全拇指脱套伤的临床疗效。方法采用髂腹股沟带蒂皮瓣修复12例全拇指脱套伤患者。缺损面积7 cm×5 cm~9 cm×6 cm。皮瓣设计面积8 cm×6 cm~11 cm×7 cm。供区直接拉拢缝合。结果患者均获得随访,时间6~18个月。12例皮瓣全部成活。1例皮瓣臃肿,二期行皮瓣修薄术;其余11例皮瓣外观无明显臃肿,质地良好,颜色与周围组织十分相近。末次随访时,患指功能恢复情况良好,拇指屈曲20°~45°、外展35°~40°。结论采用髂腹股沟带蒂皮瓣修复全拇指脱套伤方法简单易行,拇指功能恢复良好。Objective To explore the clinical effect of the ilioinguinal pedicle flap in the repair of full thumb degloving injuries.Methods A total of 12 patients with full thumb degloving injuries were repaired with ilioinguinal pedicle flaps.Defect area was 7 cm×5 cm~9 cm×6 cm.Flap designed area was 8 cm×6 cm~11 cm×7 cm.The supply area was directly pulled and sutured.Results All patients were followed up for 6~18 months.All 12 flaps achieved successful graft survival.In one case,the flap was swelling,flap thinning surgery was performed at the second stage;in the remaining 11 cases of flaps,the appearance were not significant swelling,displayed favorable texture,and closely matched the surround tissue in terms of coloration.At the final follow-up,functional recovery of the affected thumb was good,thumb flexion was 20°~45°,abduction was 35°~40°.Conclusions The ilioinguinal pedicle flap for repairing full thumb degloving injuries is a simple method,with good recovery of the thumb function.
