摘  要:目的分析磁共振(MR)弥散张量成像(DTI)的表观扩散系数(ADC)、各向异性分数(FA)与神经根型颈椎病患者颈部疼痛程度的关系。探讨神经根型颈椎病患者疼痛程度与颈部肌肉功能的相关性。方法收集2022年01月至2022年12月在我院住院治疗的符合诊断标准的神经根型颈椎病患者60例,随机分为试验组和对照组各30例,利用MR-DTI技术观察治疗前后患者颈部肌群在MR-DTI技术下的ADC值、FA值的变化,以疼痛视觉模拟量评分的变化情况判断患者颈部疼痛的程度,采用相关性研究分析ADC值、FA值的变化与神经根型颈椎病患者疼痛之间是否存在关联性。结果治疗后的FA值较治疗前显著增高(胸锁乳突肌分别为0.538±0.109和0.414±0.104,头夹肌分别为0.444±0.109、0.444±0.101)(P<0.05)、ADC值显著降低(胸锁乳突肌分别为1000.1±90.8、1336.7±117.7,头夹肌分别为1038±83.7、1356.5±98.1)(P<0.05);疼痛视觉模拟量评分显著降低[分别为(3.50±0.78)分和(7.23±1.04)分],差异有统计学意义(P<0.05)。结论神经根型颈椎病患者颈部肌群FA、ADC与疼痛程度具有显著相关性,疼痛程度与FA值呈负相关,与ADC值呈正相关。Objective To analyze the relationship between the apparent diffusion coefficient(ADC)and the fractional anisotropy(FA)of magnetic resonance(MR)diffusion tensor imaging(DTI)and the degree of neck pain in patients with cervical radiculopathy.To investigate the correlation between pain level and neck muscle function in patients with cervical radiculopathy.Methods A total of 60 patients with cervical radiculopathy who met the diagnostic criteria who were hospitalized in our hospital from January 2022 to December 2022 were randomly divided into 30 cases in the experimental group and the control group,and the ADC value and FA value of the patient's neck muscle group under MR-DTI technology before and after treatment were observed,and the degree of neck pain was judged by the change of pain visual analogue score,and the ADC value and ADC value were analyzed by correlation research.Whether there is an association between changes in FA values and pain in patients with cervical radiculopathy.Results FA value of sternocleidomastoid muscle and splenius capitis muscle were 0.538±0.109,0.414±0.104(after and before treatment),0.571±0.109,0.444±0.101(after and before treatment),respectively;ADC value of sternocleidomastoid muscle and splenius capitis muscle were 1000.1±90.8,1336.7±117.7(after and before treatment),1038.7±83.7,1356.5±98.1(after and before treatment),respectively.The FA value before and after treatment was significantly increased(P<0.05)and the ADC value was significantly reduced(P<0.05).The VAS scores before and after treatment were(7.23±1.04)and(3.50±0.78),respectively.The pain visual analogue score was significantly reduced,and the difference was statistically significant(P<0.05).Conclusion FA and ADC of cervical radiculopathy are significantly correlated with pain degree,and pain degree is negatively correlated with FA value and positive correlation with ADC value.
