基于“肺肠相关”理论探讨通腑泻下法治疗重症急性胰腺炎急性肺损伤的作用机制

摘　　要：急性肺损伤是重症急性胰腺炎的常见并发症之一,病情凶险,致死率高,常规对症治疗效果欠理想。基于“肺肠相关”理论,采用以大承气汤为代表的通腑泻下法治疗重症急性胰腺炎急性肺损伤效果显著,但其机制尚不明确。本文分别从中西医角度论述肺肠在生理病理上的相关性,阐述“肺肠相关”的理论来源和研究进展,并以此为基础论述重症急性胰腺炎并发急性肺损伤“肠病及肺”病机、“肺病治肠”治则及“通腑泻下”治法,探讨大承气汤治疗重症急性胰腺炎急性肺损伤的作用机制。Acute lung injury is one of the common complications of severe acute pancreatitis,which is dangerous and has a high mortality,and the efficacy of conventional symptomatic treatment is average.Based on the theory of“lung and intestine are related”,the treatment with the Tongfu Xiexia therapy represented by Dachengqi Decoction is remarkable,but the mechanism is still unclear.This article discussed the physiopathological correlation between lung and intestine from the perspectives of TCM and Western medicine,expounded the theoretical sources and research progress of“lung and intestine are related”,and discussed the pathogenesis of“intestinal disease involving lung”in acute lung injury complicated by severe acute pancreatitis on this basis,the treatment principle of“treating the intestines with lung disease”and the treatment method of“Tongfu Xiexia”therapy,and discussed the mechanism of Dachengqi Decoction in treating acute lung injury in severe acute pancreatitis.
