某医院注射用那屈肝素钙使用合理性分析

摘　　要：目的了解我院注射用那屈肝素钙的临床使用情况,分析用药合理性,促进临床合理用药。方法通过HIS系统收集医院2022年4月使用注射用那屈肝素钙的住院患者信息,收集其用药量、疗程等资料,分析用药合理性。结果那屈肝素钙在临床科室使用广泛,此次抽取病例涉及14个科室,23个病区,骨科术后和心血管疾病患者较多。在抽查的120份病历中,合理的75份,合理率为62.5%;点评结果为不合理的45份,不合理率为37.5%,主要包括给药频次不适宜、无理由随意更换药物、给药剂量不适宜等。结论医院那屈肝素钙使用合理率偏低,主要原因在于医护对于那屈肝素钙的了解程度不够,需要采取措施加强监管,同时加强培训学习。临床药师需要与临床医生共同努力,提高认识,促进个体化、精细化应用那屈肝素钙,使患者得到安全有效的治疗。Objective To understand the clinical use of injectable natroparin calcium in a hospital;To analyze the rationality of drug use,and promote the clinical rational use of drugs.Methods The information of inpatients who used injectable nadroparin calcium in our hospital in April 2022 was collected through HIS system,and the dosage and course of treatment were collected to analyze the rationality of drug use.Results Nadroparin calcium was widely used in clinical departments.The selected cases involved 14 departments and 23 wards,and there were more patients involved after orthopedic surgery and cardiovascular diseases.Among the 120 medical records sampled,75 were reasonable,with a reasonable rate of 62.5%;45 reviews were unreasonable,with an unreasonable rate of 37.5%,mainly including inappropriate frequency of administration,unreasonable drug replacement,inappropriate dosage,etc.Conclusion The rational use rate of nadroparin calcium in hospitals is low,mainly because doctors and nurses don't know enough about nadroparin calcium.Therefore,measures should be taken to strengthen supervision and training.Clinical pharmacists need to work together with clinicians to raise awareness and promote individualized and refined application of nadroparin calcium,so that patients can get safe and effective treatment.
