摘　　要：目的探讨Olympus CV-290SL型无痛胃肠镜在上消化道出血患者中的治疗效果及对凝血功能的影响。方法选择2021年6月—2023年2月上海电力医院收治的上消化道出血患者96例为对象,根据治疗方法不同分为两组,各48例。对照组接受常规药物止血方案,观察组在Olympus CV-290SL型无痛胃肠镜辅助下完成金属钛夹止血,均治疗7d。评估两组患者疗效、止血效果、恢复情况、炎性因子及凝血功能。结果观察组治疗总有效率为95.83%,高于对照组83.33%(P<0.05)。观察组出血停止、肠鸣音恢复、腹痛缓解及住院时间短于对照组,输血量低于对照组(P<0.05)。干预7d后,观察组IL-6、IL-17、TNF-α及CRP水平低于对照组(P<0.05)。干预7d后,观察组PT、APTT低于对照组,FIB及D-D水平高于对照组(P<0.05)。结论Olympus CV-290SL型无痛胃肠镜下金属钛夹用于上消化道出血治疗,能提高患者止血效果,利于止血后恢复,可降低炎性因子水平,改善凝血功能,值得推广应用。Objective To investigate the therapeutic effect of Olympus CV-290SL painless gastroenteroscopy in patients with upper gastrointestinal bleeding and its influence on coagulation function.Methods A total of 96 patients with upper gastrointestinal hemorrhage from June 2021 to February 2023 were selected and divided into two groups according to different treatment methods,48 cases in each group.The control group received conventional drug hemostasis,and the observation group received metal titanium clamp hemostasis with the assistance of Olympus CV-290SL painless gastroenteroscope,all of which were treated for 7 days.The therapeutic effect,hemostatic effect,recovery,inflammatory factors and coagulation function of the two groups were evaluated.Results The total effective rate of the observation group was 95.83%,higher than that of the control group 83.33%(P<0.05).The bleeding stop,bowel sound recovery,abdominal pain relief and hospital stay in the observation group were shorter than those in the control group,and the blood transfusion volume was lower than that in the control group(P<0.05).After 7 days of intervention,the levels of IL-6,IL-17,TNF-αand CRP in observation group were lower than those in control group(P<0.05).After 7 days of intervention,PT and APTT in observation group were lower than those in control group,and FIB and D-D levels were higher than those in control group(P<0.05).Conclusion Olympus CV-290SL type painless gastroenteroscopy metal titanium clip for the treatment of upper gastrointestinal bleeding can improve the hemostatic effect of patients,facilitate the recovery after hemostasis,reduce the level of inflammatory factors,improve coagulation function,and is worthy of promotion and application.
