摘 要:目的比较不同体质量指数(BMI)多囊卵巢综合征(PCOS)患者的临床特点和糖脂代谢的差异,探究肥胖型PCOS患者的糖脂代谢异常的特点。方法选择2016年1月—2022年12月绍兴市妇幼保健院生殖中心就诊的PCOS患者230例为研究对象,根据体质量指数分为2组:BMI≥25 kg/m^(2)为肥胖组(A组),BMI<25 kg/m^(2)为非肥胖组(B组)。对2组患者年龄、WHR、OGTT试验的血糖、胰岛素和HOMA进行比较分析。以SPSS 19.0软件进行统计分析。结果肥胖组比非肥胖组BMI、WHR、血脂3项(TC、TG、LDL)、空腹血糖和胰岛素水平、OGTT后0.5 h血糖和胰岛素水平、OGTT后1 h血糖和胰岛素水平、OGTT后2 h血糖和胰岛素水平、OGTT后3 h胰岛素水平以及HOMA-IR均高度显著增加(P<0.01),肥胖组比非肥胖组OGTT后3 h血糖水平明显增加(P<0.05),血脂HDL则显著下降(P<0.01)。肥胖组的胰岛素抵抗率、糖尿病率和血脂异常率均显著高于非肥胖组,差异均有统计学意义(P<0.01)。结论肥胖型PCOS患者有更为严重的糖脂代谢紊乱,应尽早进行血糖和血脂检查以便及时干预,从而预防和延缓PCOS的远期并发症。Objective This paper aims to compare the clinical characteristics of polycystic ovary syndrome(PCOS)and the differences of glucose and lipid metabolism in patients with different body mass index(BMI),so as to explore the characteristics of abnormal glucose and lipid metabolism in obese PCOS patients.Methods A total of 230 PCOS patients who visited the reproductive center of Shaoxing Maternal and Child Health Care Hospital from January 2016 to December 2022 were selected as the research subjects,and they were divided into 2 groups according to body mass index:those with BMI≥25 kg/m^(2)were classified as obese group(Group A),and those with BMI<25 kg/m^(2)were classified as non-obese group(Group B).Age in WHR,blood glucose,insulin and HOMA and OGTT tests were compared between the two groups.Statistical analysis was carried out with SPSS 19.0 software.Results Compared with the non-obese group,BMI,WHR,blood lipids(TC,TG,LDL),fasting blood glucose and insulin levels,blood glucose and insulin levels at 0.5 h after OGTT,blood glucose and insulin levels at 1 h and 2 h after OGTT,insulin levels at 3 h after OGTT,and HOMA-IR of the obese group were significantly increased(P<0.01).Compared with the non-obese group,the blood glucose level in the obese group was significantly increased at 3 h after OGTT(P<0.05),while the blood lipid HDL was significantly decreased(P<0.01).The rates of insulin resistance,diabetes and dyslipidemia in the obese group were significantly higher than those in the non-obese group(P<0.01).Conclusion Obese patients with PCOS had more severe glucose and lipid metabolism disorders.Blood glucose and blood lipids should be checked as soon as possible for timely intervention in order to prevent and delay the long-term complications of PCOS.
