摘 要:目的考察T-SPOT.TB联合INF-γ、IL-2检测在结核病诊断中的价值。方法选择2020年1月-2022年12月就诊于本院的疑似结核病患者为研究组(138例),以健康体检者为对照组(130例),考察2组T-SPOT.TB阳性率、INF-γ和IL-2水平、T-SPOT.TB联合INF-γ和IL-2的诊断价值。结果果本研究中T-SPOT.TB诊断肺结核阳性122例,阳性率为88.4%。研究组INF-γ水平为(117.28±24.01)pg/ml,高于对照组的(90.31±5.45)pg/ml,研究组IL-2水平为(522.31±106.21)pg/ml,高于对照组的(452.67±86.27)pg/ml差异均有统计学意义(P<0.05)。采用T-SPOT.TB联合INF-γ和IL-2对2组进行诊断,T-SPOT.TB与INF-γ和IL-2分别联合诊断结核病均有较好的效果,其中T-SPOT.TB同时联合INF-γ和IL-2的AUC最大,为0.934,且特异度最高,敏感度略低于TSPOT.TB联合INF-γ或T-SPOT.TB联合IL-2。结论T-SPOT.TB联合INF-γ、IL-2检测结核病中具有较好的诊断效果,敏感度和特异度均较高。Objective This paper aims to investigate the value of T-SPOT.TB combined with interferon-(INF-γ)and interleukin-2(IL-2)in the diagnosis of tuberculosis.Methods From January 2020 to December 2022,138 suspected tuberculosis patients in our hospital were selected as the study group and 130 healthy people as the control group.The positive rate of T-SPOT.TB,INF-γand IL-2 levels,and the diagnostic value of T-SPOT.TB combined with INF-γand IL-2 were investigated.Results In this study,122 cases were positive for T-SPOT.TB,with a positive rate of 88.4%.The INF-γlevel of the study group was(117.28±24.01)pg/ml,which was higher than the control group[(90.31±5.45)pg/ml],and the IL-2 level of the study group was(522.31±106.21)pg/ml,higher than the control group[(452.67±86.27)pg/ml],and the differences were statistically significant(P<0.05).T-SPOT.TB combined with INF-γand IL-2 was used to diagnose the two groups.T-SPOT.TB combined with INF-γand IL-2 respectively had good results in the diagnosis of tuberculosis.The combination of T-SPOT.TB with INF-γand IL-2 has the largest AUC(O.934)and the highest specificity.The sensitivity of the combined detection was slightly lower than that of T-SPOT.TB combined with INF-γor IL-2.Conclusion T-SPOT.TB combined with INF-γand IL-2 has a good diagnostic effect in detecting tuberculosis,with high sensitivity and specificity.
