摘  要:目的 了解湖北省荆州市严重急性呼吸道感染(sever acute respiratory infection,SARI)住院病例的呼吸道病毒感染情况,分析病毒病原谱特征。方法 收集2016—2020年湖北省荆州市哨点医院SARI病例咽拭子样本,采用实时荧光定量PCR方法检测流感病毒、人鼻病毒、人腺病毒、人副流感病毒、人呼吸道合胞病毒、人偏肺病毒、人冠状病毒及人博卡病毒8种常见呼吸道病毒,分析SARI病原谱构成、季节分布规律及易感人群。结果 共收集并检测SARI病例样本1 506份,阳性检出率为55.11%,8种病原均有检出,其中病原谱前三的病毒为流感病毒(15.34%)、人呼吸道合胞病毒(9.83%)及人腺病毒(6.44%)。病毒混合感染率为7.97%,以人鼻病毒或人腺病毒合并感染最常见。病毒检出高峰在秋季(63.27%)和冬季(58.54%),不同的病毒呈现出其特征性季节流行规律,冬春季节主要流行流感病毒及人呼吸道合胞病毒,夏季为人腺病毒,秋季为人鼻病毒。5岁以下婴幼儿(60.69%)发病率高于成人。结论 荆州市SARI病毒性病原谱构成复杂,人呼吸道合胞病毒、人腺病毒及流感病毒是荆州市近五年SARI病例感染的主要病毒类病原体,且不同季节病原谱构成不同,应加强对上述呼吸道病毒的监测和研究,为本地SARI防控提供实验室依据。Objective To investigate the respiratory tract virus infection of severe acute respiratory infection(SARI)in Jingzhou city,Hubei Province,and to analyze the characteristics of viral pathogen spectrum.Methods Throat swab samples of SARI cases in Jingzhou Sentinel Hospital of Hubei Province from 2016 to 2020 were collected.Eight common respiratory viruses including influenza virus,human rhinovirus,human adenovirus,human parainfluenza virus,human respiratory syncytial virus,human metapneumovirus,human coronavirus,and human bocavirus were detected by real-time fluorescence quantitative PCR.The pathogen spectrum,seasonal distribution,and susceptible population of SARI were analyzed.Results A total of 1506 SARI case samples were collected and tested,and the positive rate was 55.11%.All 8 pathogens were detected,of which the top three viruses in the pathogen spectrum were influenza virus(15.34%),human respiratory syncytial virus(9.83%),and human adenovirus(6.44%).The mixed infection rate of virus was 7.97%,and the most common infection was human rhinovirus or human adenovirus.The peak of virus detection was in autumn(63.27%)and winter(58.54%).Different viruses showed their characteristic seasonal epidemic regularities.Influenza virus and human respiratory syncytial virus were prevalent in winter and spring,human adenovirus in summer and human rhinovirus in autumn.The incidence of infants under 5 years old(60.69%)was higher than that of adults.Conclusion The pathogen spectrum of SARI virus in Jingzhou is complex.Human respiratory syncytial virus,human adenovirus,and influenza virus are the main viral pathogens of SARI infection in Jingzhou in recent five years,and the pathogen spectrum is different in different seasons.Surveillance and research on the above respiratory viruses should be strengthened to provide laboratory basis for local SARI prevention and control.
