摘 要:目的探讨外周血细胞因子白细胞介素-33(interleukin-33)在学龄前儿童反复喘息发作中的作用。方法选择2018年2-7月于本院住院治疗的80例学龄前反复喘息患儿(喘息组)以及40名健康儿童(对照组)作为研究对象,采用流式细胞仪检测外周血CD4^(+)CD25^(+)CD127^(low)Treg细胞百分比,采用ELISA法检测各组外周血血清IL-33、IL-17、IL-10、转化生长因子-β_(1)(Transforming growth factor-β_(1))水平,并进行比较分析。结果喘息组患儿外周血Treg细胞百分比及IL-10、TGF-β1水平较对照组均降低(P<0.05),IL-33及IL-17水平较对照组均升高(P<0.05);在喘息组中,血清IL-33水平与外周血调节性T细胞(T regulatory cells)比例及血清IL-10、TGF-β_(1)水平呈负相关(r=-0.2721,r=-0.2586,r=-2898,P<0.05),与IL-17水平呈正相关(r=0.3779,P<0.05)。在对照组中,血清IL-33水平与外周血Treg细胞比例及血清IL-10、TGF-β_(1)、IL-17水平均无相关性(P>0.05)。结论细胞因子IL-33可能参与了学龄前儿童反复喘息的发病机制,并与Treg细胞及细胞因子IL-10、TGF-β_(1)、IL-17间可能存在相互作用。Objective To investigate the role of peripheral blood cytokine interleukin-33(IL-33)in recurrent wheezing attacks in preschool children.Methods 80 preschool children with recurrent wheezing(wheezing group)hospitalized in our hospital from February 2018 to July 2018and 40 healthy children(control group)were enrolled as study subjects,flow cytometry was used to detect the percentage of CD4^(+)CD25^(+)CD127^(low)Treg cells in peripheral blood;and the serum IL-33,IL-17,IL-10 and transforming growth factor-β1(TGF-β1)level were detected by ELISA,and conducted comparative analysis.Results The percentage of peripheral blood Treg cells,and the levels of IL-10 and TGF-β1 in wheezing group were lower than those in the control group(P<0.05),and the levels of IL-33 and IL-17 were higher than those in the control group(P<0.05).A negative correlation was found between the level of serum IL-33 and the ratio of peripheral blood Treg cells,the levels of serum IL-10 and TGF-β1 in preschool children with recurrent wheezing(r=-0.2721,r=-0.2586,r=-2898,P<0.05);but a positive correlation between the levels of serum IL-33 and IL-17(r=0.3779,P<0.05).In the control group,there was no correlation between serum IL-33 level and the ratio of peripheral blood Treg cells and serum IL-10,TGF-β1,IL-17 levels(P>0.05).Conclusions Cytokine IL-33 may be involved in the pathogenesis of recurrent wheezing in preschool children,and may interact with Treg cells and cytokines IL-10,TGF-β1 and IL-17.
