自制足浴方泡足联合艾灸治疗糖尿病足患者的效果及对患肢疼痛情况的影响

摘　　要：目的 观察自制足浴方泡足联合艾灸治疗糖尿病足患者的效果及对患肢疼痛情况的影响分析。方法选取2017年1月-2023年1月枝江市中医医院收治的糖尿病足患者100例为研究对象,根据治疗方法的不同分为两组,以常规基础治疗+清创换药为对照组(50例),在对照组基础上配合自制足浴方泡足联合艾灸治疗为研究组(50例)。比较两组患者在治疗前后不同时间段的疼痛评分、疮疡面积、血清炎症因子水平、血流动力学及踝肱指数指标,判定治疗效果。结果 两组治疗后1、2、3周的疼痛评分、疮疡面积均优于治疗前,差异有统计学意义(P<0.05)。两组治疗后的血清炎症水平均较治疗前下降,且研究组低于对照组,而足背动脉血管内径、血流动力学及踝肱指数指标较治疗前提升,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论 在糖尿病足患者的治疗中,以自制足浴方泡足联合艾灸治疗,可显著降低患者在治疗后的疼痛严重程度,缩小疮疡面积,降低血清炎性指标水平,改善足背动脉血管内径、血流动力学及踝肱指数,提高治疗总有效率,效果理想。Objective To observe the effect of foot bath recipe combined with moxibustion on diabetic foot patients and the effect on the pain of affected limbs.Methods 100 patients with diabetes foot who were admitted to Zhijiang Hospital of Traditional Chinese Medicine from January 2017 to January 2023 were selected as the research objects.According to the different treatment methods,they were divided into two groups.The control group(50 cases)was con-ventional basic treatment+debridement and dressing change.On the basis of the control group,the research group(50 cases)was treated with self-made foot bath formula combined with moxibustion.Compared the pain scores,sore area,serum inflammatory factor levels,hemodynamics,and ankle brachial index indicators of two groups of patients at dif-ferent time periods before and after treatment to determine the treatment effect.Results The pain scores and ulcer area of the two groups at 1,2,and 3 weeks after treatment were better than before treatment,the difference was statisti-cally significant(P<0.05).After treatment,the serum inflammatory levels in both groups decreased compared to before treatment,and the study group was lower than the control group.However,the indicators of dorsal foot artery vessel di-ameter,hemodynamics,and ankle brachial index increased compared to before treatment,and the study group was higher than the control group,the difference was statistically significant(P<0.05).The total effective rate of treatment in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion In the treatment of diabetic foot patients,foot bath combined with moxibustion can significantly reduce the severity of pain after treatment,reduce the sore area,reduce the level of serum inflammatory indexes,improve the internal diameter of dorsal foot artery,hemodynamics and ankle-brachial index,and improve the total effective rate.
