血栓弹力图对低密度脂蛋白升高的2型糖尿病患者凝血功能的评价分析

摘　　要：目的 探讨血栓弹力图(thromboelastogram, TEG)在评估低密度脂蛋白(low density lipoprotein, LDL)水平升高的2型糖尿病(type 2 diabetes mellitus, T2DM)患者凝血功能中的效果。方法 选取自2021年12月-2022年12月于浙江大学医学院附属第二医院健康管理中心进行检查的131例2型糖尿病成人患者,其中105例LDL未达标的患者定义为LDL升高(type 2 diabetes mellitus with high LDL, T2DM-HL)组,26例LDL达标组的患者定义为LDL正常(type 2 diabetes mellitus with normal LDL, T2DM-NL)组,收集所有患者的一般情况以及血常规、TEG以及生化指标,比较两组间TEG指标的差异,分析血栓弹力图参数与LDL指标的相关性。结果 本研究共纳入131例T2DM患者,以1.8 mmol/作为LDL分组的临界点,将患者分为T2DM-NL组(26例)和T2DM-HL组(105例),结果显示,两组患者各指标比较,差异无统计学意义(P>0.05)。与T2DM-NL组相比,T2DM-HL组患者的R值和K值显著下降,而Ma、α角和CI值则显著增高,差异有统计学意义(P<0.05)。采用Spearman相关分析结果显示,LDL指标与α角、Ma、CI值显著正相关(P<0.05),与R、K值显著负相关(P<0.05)。结论 LDL水平升高的T2DM患者的TEG指标会出现显著的变化(R值、K值降低,α/°、MA值和CI值升高),提示TEG可以用来评估LDL水平增加的T2DM患者的凝血功能。Objective To investigate the effect of thromboelastogram(TEG)in evaluating coagulation function in pa-tients with type 2 diabetes mellitus(T2DM)with elevated low density lipoprotein(LDL)levels.Methods 131 adult pa-tients with type 2 diabetes who were examined at the Health Management Center of the Second Affiliated Hospital of Zhejiang University School of Medicine from December 2021 to December 2022 were selected.Among them,105 pa-tients who did not reach the LDL standard were defined as the type 2 diabetes mellitus with high LDL(T2DM-HL)group,and 26 patients who reached the LDL standard were defined as the type 2 diabetes mellitus with normal LDL(T2DM-NL)group,Collect the general information,blood routine,TEG,and biochemical indicators of all patients,compare the differences in TEG indicators between the two groups,and analyze the correlation between thromboelasto-gram parameters and LDL indicators.Results A total of 131 T2DM patients were included in this study,and 1.8 mmol/was used as the threshold for LDL grouping.The patients were divided into the T2DM-NL group(26 cases)and the T2DM-HL group(105 cases).The results showed that there was no statistically significant difference in the indicators between the two groups(P>0.05).Compared with the T2DM-NL group,the R and K values of patients in the T2DM-HL group significantly decreased,while MaαThe angle and CI values significantly increased,the difference was sta-tistically significant(P<0.05).The results of Spearman correlation analysis show that the LDL index is related toαThe angle,Ma,and CI values were significantly positively correlated(P<0.05),and negatively correlated with R and K values(P<0.05).Conclusion In T2DM patients with elevated LDL levels,there are significant changes in TEG mark-ers(decreased R and K values,and increasedα/°,MA,and CI values),suggesting that TEG can be used to evaluate coagulation function in T2DM patients with increased LDL levels.
