甲状腺功能减退对妊娠期糖尿病孕妇的影响分析

摘　　要：目的探讨甲状腺功能减退对妊娠期糖尿病孕妇的影响。方法选取2019年10月—2022年12月泗洪医院收治的妊娠期糖尿病患者40例,按照甲状腺功能是否正常,分为两组,各20例。患甲状腺功能减退症的妊娠期糖尿病患者为观察组,甲状腺功能正常的妊娠期糖尿病患者为对照组。对比两组患者各项指标。结果观察组总胆固醇、三酰甘油、低密度脂蛋白胆固醇与甲状腺激素水平高于对照组,差异有统计学意义(P<0.05)。观察组纤维蛋白原高于对照组,抗凝血酶Ⅲ低于对照组,差异有统计学意义(P<0.05)。观察组超敏C反应蛋白、肿瘤坏死因子、白细胞介素-6均高于对照组,差异有统计学意义(P<0.05)。观察组空腹胰岛素、胰岛素抵抗指数水平均高于对照组,胰岛素敏感性指数水平低于对照组,差异有统计学意义(P<0.05)。结论对于甲状腺功能减退对妊娠期糖尿病孕妇糖代谢孕妇,其凝血纤溶系统、脂质代谢系统存在明显异常,慢性炎症症状表现突出,且甲状腺功能减退症状会导致机体胰岛素抵抗程度升高,胰岛素敏感性下降。Objective To investigate the effect of hypothyroidism on pregnant women with gestational diabetes mellitus.Methods 40 patients with gestational diabetes were treated in Sihong Hospital from October 2019 to December 2022 were slected,and divided into two groups according to whether the thyroid function was normal,20 cases in each.Gestational diabetes with hypothyroidism were the observation group,and gestational diabetes with normal thyroid function were the control group.Compared the indicators between the two groups of patients.Results The levels of TC,TG,LDL-C,and TSH in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The FIB of the observation group was higher than that of the control group,and the AT III was lower than that of the control group,the difference was statistically significant(P<0.05).The hs-CRP,TNF-α,IL-6 of observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).The levels of INS and HOMA-IR in the observation group were higher than those in the control group,while ISI level was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion For pregnant women with gestational diabetes induced by hypothyroidism and glucose metabolism,there are obvious abnormalities in the coagulation and fibrinolytic system and lipid metabolism system,and the symptoms of chronic inflammation are prominent,and the symptoms of hypothyroidism will lead to increased insulin resistance and decreased insulin sensitivity.
