摘　　要：目的分析常规抗心衰、基于达格列净的降糖治疗方案对心衰伴糖尿病患者体质量波动情况及心功能的影响。方法选取2020年1月—2022年1月福建省建瓯市立医院收治的105例心力衰竭伴糖尿病患者为研究对象,按照不同治疗方式将其分成两组,对照组50例患者接受常规抗心衰及降糖(不含钠-葡萄糖协同转运蛋白2抑制剂)疗法,研究组55例患者接受常规抗心衰及基于达格列净的联合降糖疗法。对比两组的体质指数波动情况、血糖控制水平及心功能指标变化情况。结果治疗后,研究组的体质指数低于对照组,差异有统计学意义(P<0.05)。治疗后两组的空腹血糖、餐后2 h血糖及糖化血红蛋白水平比较,差异无统计学意义(P>0.05)。研究组的左心室射血分数、左心室舒张末内径水平显著高于对照组,心率水平低于对照组,差异有统计学意义(P<0.05)。研究组的不良反应发生率及半年再住院率明显低于对照组,差异有统计学意义(P<0.05)。结论心衰伴糖尿病患者在接受常规抗心衰基础上给予基于达格列净的联合降糖治疗能有效降低患者体质量,对其明显改善患者心功能,减少心血管不良事件及再住院率。Objective To analyze the effects of anti-heart failure and dapagliflozin based hypoglycemic therapy on weight fluctuation and cardiac function in patients with heart failure and diabetes.Methods From January 2020 to January 2022,105 patients with heart failure and diabetes mellitus admitted to Jian'ou Municipal Hospital of Fujian Province were selected as the research objects.They were divided into two groups according to different treatment methods.50 patients in the control group received conventional anti-heart failure and hypoglycemic therapy(excluding sodium-glucose cotransporter2 inhibitors),and 55 patients in the study group received conventional anti-heart failure and dapagliflozin-based combined hypoglycemic therapy.The changes of body mass index,blood glucose control level and cardiac function index were compared between the two groups.Results After treatment,the body mass index of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin levels between the two groups after treatment(P>0.05).The left ventricular ejection fraction and left ventricular end-diastolic diameter of the study group were significantly higher than those of the control group,and the heart rate level was lower than that of the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions and half-year rehospitalization rate in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion In patients with heart failure complicated with diabetes,the combination hypoglycemic therapy based on dapagliflozin on the basis of conventional anti-heart failure can effectively reduce the body weight of patients,significantly improve the heart function of patients,reduce cardiovascular adverse events and the rate of re-hospitalization,w
