甘精胰岛素联合谷赖胰岛素治疗糖尿病合并食管癌围术期的效果分析

摘　　要：目的分析甘精胰岛素联合谷赖胰岛素治疗糖尿病合并食管癌围术期的效果。方法选取2019年11月—2022年10月泉州市第一医院收治的64例糖尿病合并食管癌患者为研究对象,以随机数表法分成两组,每组32例。对照组采用赖脯胰岛素联合甘精胰岛素治疗,研究组采用谷赖胰岛素联合甘精胰岛素治疗。比较两组患者在治疗前后的血糖水平、日需胰岛素剂量、低血糖发生次数和围术期并发症发生情况。结果治疗后两组的空腹、餐后2 h、午餐前、午餐后2 h、晚餐前、晚餐后2 h及睡前的血糖水平较治疗前明显下降,且研究组明显低于对照组,差异有统计学意义(P<0.05)。研究组患者的日需胰岛素剂量及低血糖发生次数显著少于对照组,差异有统计学意义(P<0.05)。研究组患者在围术期发生并发症率低于对照组,差异有统计学意义(P<0.05)。结论糖尿病合并食管癌围术期患者,以精胰岛素联合谷赖胰岛素治疗,可显著控制患者的血糖指标水平,减少日需胰岛素剂量及低血糖发生次数,降低围术期并发症发生率,效果理想。Objective To observe the perioperative effect of insulin glargine combined with insulin glistin in the treatment of diabetes mellitus complicated with esophageal cancer.Methods A total of 64 patients with diabetes mellitus complicated with esophageal cancer admitted to the First Hospital of Quanzhou from November 2019 to October 2022 were selected as the research objects.They were divided into two groups by random number table method,with 32 cases in each group.The control group was treated with insulin lispro combined with insulin glargine,and the study group was treated with insulin glulisine combined with insulin glargine.The blood glucose level,daily insulin dose,frequency of hypoglycemia and perioperative complications were compared between the two groups before and after treatment.Results After treatment,the blood glucose levels of fasting,2 h after meal,before lunch,2 h after lunch,before dinner,2 h after dinner and before bedtime in the two groups were significantly lower than those before treatment,and those in the study group were significantly lower than the control group,the differences were statistically significant(P<0.05).The daily insulin dose and the number of hypoglycemia in the study group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of perioperative complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion In patients with diabetes mellitus complicated with esophageal cancer during perioperative period,insulin progenitor combined with insulin glistin can significantly control the blood glucose index level of patients,reduce the daily insulin dose and the frequency of hypoglycemia,and reduce the incidence of perioperative complications.The effect is ideal and worthy of clinical promotion.
