基于瘀血学说观察血府逐瘀汤治疗不稳定型心绞痛临床效果

摘　　要：目的基于中医瘀血学说观察血府逐瘀汤对不稳定型心绞痛(UAP)患者心功能及血液流变学的影响。方法将符合标准的60例患者随机分为对照组(30例)和治疗组(30例),均给予常规治疗,治疗组加用血府逐瘀汤,比较2组治疗前后发作情况、心电图疗效、心功能相关指标、血液流变学相关指标。结果治疗后,治疗组心电图总有效率90.00%(27/30)高于对照组的50.00%(15/30)(P<0.05);2组患者左心室射血分数(LVEF)、心排血量均升高,左心室收缩末期内径(LVESD)均缩小(P<0.05),血浆黏度(PV)、全血高切黏度(NBH)、全血低切黏度(NBL)、纤维蛋白原(FIB)均降低(P<0.05),且上述指标治疗组改善更明显(P<0.05)。结论血府逐瘀汤能够有效改善UAP患者心功能与血液流变学指标,临床疗效确切,值得借鉴。Objective To observe the effect of Xuefu Zhuyu decoction on cardiac function and hemorheology of patients with unstable angina pectoris based on the theory of blood stasis in traditional Chinese medicine.Methods Sixty patients who met the criteria were randomly divided into control group of 30 cases and treatment group of 30 cases.All received conventional treatment,the treatment group was given additionally Xuefu Zhuyu decoction.The seizure,electrocardiogram,cardiac function and hemorheology were compared between the two groups before and after treatment.Results After treatment,the total effective rate of electrocardiogram in treatment group was 90.00%(27/30),which was higher than that in control group 50.00%(15/30)(P<0.05).Left ventricular ejection fraction(LVEF)and cardiac output increased,left ventricular end-systolic inner diameter(LVESD)decreased(P<0.05),plasma viscosity(PV),whole blood high tangential viscosity(NBH),whole blood low tangential viscosity(NBL)and fibrinogen(FIB)decreased(P<0.05),and the above indexes were improved in the treatment group obviously(P<0.05).Conclusion Xuefu Zhuyu decoction can effectively improve the cardiac function and hemorheology indexes of patients with unstable angina pectoris,has definite clinical efficacy,and is worthy of reference.
