摘　　要：目的探讨超声引导下针刀松解疗法对肩峰下撞击综合征(SIS)患者的临床效果。方法纳入80例临床期SIS患者,按照随机数字表法分为观察组(行超声引导下针刀松解疗法)40例及对照组(行常规针刺)40例,对照组单纯针刺治疗,观察组在对照组的基础上行超声实时引导下针刀松解疗法。记录2组总体临床疗效,治疗前后视觉模拟量表(VAS)评分、Constant肩关节评分,治疗前后三大常规、肝功能、电解质、心电图检查结果等安全性指标。结果观察组的总有效率为95.00%(38/40),显著高于对照组的85.00%(34/40)(P<0.05);观察组VAS评分、Constant肩关节评分均低于对照组(P<0.05);2组安全性指标均未见明显异常。结论经临床对比,超声引导下针刀松解疗法用于SIS在总体临床疗效、VAS评分、Constant肩关节评分方面较常规针刺疗法更优。Objective To investigate the clinical efficacy of ultrasound-guided acupotomy release therapy for patients with subacromial impingement syndrome(SIS).Methods A total of 80 patients with SIS in clinical stage were included and divided into observation group of 40 cases(received ultrasound-guided acupotomy release therapy)and control group of 40 cases(received routine acupuncture treatment)according to random number table method.The control group received acupuncture treatment alone,and the observation group received ultrasound-guided acupotomy release therapy on the basis of the control group.The overall clinical efficacy,visual analogue scale(VAS)score before and after treatment,Constant shoulder joint score,three routine tests before and after treatment,liver function,electrolyte,electrocardiogram and other safety indicators were recorded in the 2 groups.Results The total effective rate of observation group was 95.00%(38/40),which was significantly higher than that of control group(85.00%,34/40)(P<0.05).VAS score and Constant shoulder joint score in observation group were lower than those in control group(P<0.05).No obvious abnormality was found in the two groups of safety indexes.Conclusion According to clinical comparison,ultrasound-guided acupotomy release therapy is superior to acupuncture alone in the overall clinical efficacy,VAS score and Constant score in the treatment of subacromial impingent syndrome.
