摘　　要：目的研究经皮自体骨髓浓集液联合富血小板血浆治疗骨折延迟愈合的临床效果。方法选取2019年1月至2020年12月南阳市中心医院收治的126例骨折延迟愈合患者,按照随机数字表法分为对照组(63例)和研究组(63例)。对照组接受经皮自体骨髓浓集液治疗,研究组在对照组基础上接受富血小板血浆治疗。对比两组骨髓血抽取量和治疗费用、不同愈合程度骨痂形成时间、视觉模拟评分法(VAS)评分和胫骨骨折影像学愈合评分系统(RUST)评分及生活质量调查表(SF-36)评分。结果与对照组比较,研究组骨髓血抽取量少、治疗费用高(P<0.05);术后24个月与对照组比较,研究组VAS评分低、RUST评分高(P<0.05);与对照组比较,研究组少量骨痂、中量骨痂、大量骨痂形成时间及临床愈合时间短(P<0.05);术后24个月与对照组比较,研究组SF-36评分高(P<0.05)。结论经皮自体骨髓浓集液联合富血小板血浆治疗骨折延迟愈合效果显著,能减少骨髓抽取量,减轻机体疼痛,促进骨折快速愈合,提升患者生活质量。Objective To investigate the clinical effect of percutaneous autologous bone marrow concentrate combined with platelet-rich plasma in the treatment of delayed union of fractures.Methods A total of 126 patients with delayed union of fractures in Nanyang Central Hospital from January 2019 to December 2020 were enrolled,and they were divided into control group(63 cases)and research group(63 cases)according to the random number table method.The control group was treated with percutaneous autologous bone marrow concentrate,and the research group was treated with platelet-rich plasma on the basis of the control group.The two groups were compared with bone marrow blood extraction and treatment cost,callus formation time with different degrees of healing,visual analogue scale(VAS)score,radiographic healing scoring system for tibial fractures(RUST)score and short form 36 questionnaire(SF-36).Results Compared with the control group,the research group had less bone marrow blood extraction and high treatment cost(P<0.05).Compared with the control group 24 months after surgery,the VAS score of the research group was lower,and RUST score of the research group was higher(P<0.05).Compared with the control group,the research group had a shorter time for a small amount of callus,a moderate amount of callus,a large amount of callus formation and clinical healing(P<0.05).Compared with the control group 24 months after surgery,the research group had a high SF-36 score(P<0.05).Conclusion Percutaneous autologous bone marrow concentrate combined with platelet-rich plasma has a significant effect in the treatment of delayed union of fractures,which can reduce the amount of bone marrow extraction,reduce body pain,promote rapid fracture healing,and improve the quality of life of patients.
