摘　　要：目的:探讨外周血miR-30a、miR-24及Sestrin2的表达水平与急性心肌梗死患者心肌损伤程度之间的相关性,以评估其在心肌梗死中的潜在临床价值。方法:选取2020年1月至2023年1月于我院心内科住院均初次确诊为急性ST段抬高型心肌梗死的患者180例为研究组,根据年龄匹配正常人100例为对照组,比较两组的外周血miR-30a、miR-24及Sestrin2表达情况,对患者行冠脉造影检查,并根据其心肌损伤程度将研究组患者分为轻度组和重度组,并检测两组间患者的与CK、CK-MB、LDH及hs-cTnI水平,探究其与外周血miR-30a、miR-24及Sestrin2表达间的相关性。结果:与对照组患者相比较,研究组患者的miR-30a相对表达量和Sestrin2水平升高,miR-24相对表达量降低(P<0.05)。与轻度组患者相比较,重度组患者的miR-30a相对表达量和Sestrin2水平升高,miR-24相对表达量降低(P<0.05)。与轻度组患者相比较,重度组患者的CK、CK-MB、LDH及hs-cTnI水平均升高(P<0.05)。外周血miR-30a相对表达量及Sestrin2水平与CK、CK-MB、LDH及hs-cTnI水平呈正相关,miR-24相对表达量与CK、CK-MB、LDH及hs-cTnI水平呈负相关(P<0.05)。以患者心肌损伤程度为变量,分析外周血miR-30a、miR-24相对表达量及Sestrin2水平对其诊断价值,各指标AUC值分别为0.942、0.989、0.862。结论:在急性心肌梗死患者中,重度心肌损伤组的miR-30a和Sestrin2表达水平更高,miR-24表达水平更低,外周血miR-30a、miR-24及Sestrin2的表达与心肌损伤程度存在显著相关性,可作为评估急性心肌梗死患者心肌损伤程度的潜在生物标志物,为急性心肌梗死的危险分层提供依据,指导临床医师对急性心肌梗死患者的临床治疗决策。Objective:To explore the correlation between the expression levels of miR-30a,miR-24 and Sestrin2 in peripheral blood and the degree of myocardial injury in AMI patients in order to evaluate its potential clinical value in myocardial infarction.Methods:The study included 180 patients who were diagnosed with acute ST-segment elevation myocardial infarction between January 2020 and January 2023.A control group consisting of 100 age-matched individuals without cardiac conditions was also established.The study compared the peripheral blood levels of miR-30a,miR-24,and Sestrin2 between the two groups.Coronary angiography was performed on the patients to assess the extent of myocardial injury.Patients in the study group were categorized into mild and severe groups based on the severity of myocardial injury.The study also analyzed the correlation between peripheral blood miR-30a,miR-24,and Sestrin2 expression and levels of cardiac markers(CK,CK-MB,LDH,and hs-cTnI).Results:Compared to the control group,patients in the study group showed increased relative expression levels of miR-30a and Sestrin2,and decreased relative expression of miR-24(P<0.05).Within the study group,patients in the severe myocardial injury group exhibited higher relative expression levels of miR-30a and Sestrin2,and lower relative expression of miR-24 compared to the mild group(P<0.05).Additionally,CK,CK-MB,LDH,and hs-cTnI levels were elevated in the severe group when compared to the mild group(P<0.05).Peripheral blood miR-30a and Sestrin2 levels were positively correlated with levels of CK,CK-MB,LDH,and hs-cTnI,while miR-24 expression was negatively correlated with these cardiac markers(P<0.05).Using the degree of myocardial injury as a variable,the study analyzed the diagnostic value of peripheral blood levels of miR-30a and miR-24 as well as Sestrin2.The AUC values for each index were 0.942,0.989,and 0.862,respectively.Conclusion:In patients with acute myocardial infarction,those with severe myocardial injury had higher levels of miR-30a and Sestr
