孕期综合保健指导护理对高危孕产妇妊娠结局的影响

摘　　要：目的分析孕期综合保健指导护理对高危孕产妇妊娠结局的实际应用效果。方法选择2022年1~12月在我单位建卡的785例高危孕产妇,随机分为两组。对照组仅接受产科门诊的常规护理干预,研究组则在对照组的基础上接受了孕期综合保健指导护理,分别比较两组产妇干预前后心理状态、妊娠、分娩期及新生儿并发症、产妇妊娠结局等。结果研究组产妇护理后SAS、SDS、PSRS评分低于对照组(P<0.05);研究组产妇发生妊娠及分娩期并发症低于对照组,新生儿发生黄疸、巨大儿、新生儿窒息发生率低于对照组(P<0.05);研究组产妇自然分娩率、胎儿Apgar评分均高于对照组,剖宫产、难产、死胎发生率低于对照组(P<0.05)。结论对高危产妇进行孕期综合保健指导护理有助于改善产妇不良心理状态,减少妊娠及分娩期并发症发生率,并对预防新生儿并发症有积极作用,有利于产妇自然分娩,减少剖宫产及巨大儿的发生,值得临床推广应用。Objective To analyze the practical application effect of comprehensive health care guidance and nursing during pregnancy on the pregnancy outcome of high-risk pregnant women.Methods 785 high-risk pregnant women from January to December 2022 were selected and randomly divided into two groups.The control group only received routine nursing intervention in the obstetric clinic,while the study group received comprehensive health care guidance and nursing during pregnancy on the basis of the control group.The psychological state,pregnancy,delivery period,neonatal complications and maternal pregnancy outcomes of the two groups were compared respectively before and after the intervention.Results The scores of SAS,SDS and PSRS in the study group were lower than those in the control group(P<0.05).The incidence of complications during pregnancy and childbirth in the study group was lower than that in the control group,and the incidence of jaundice,macrosomia and neonatal asphyxia were significantly lower in the study group than in the control group(P<0.05).The rate of spontaneous delivery and fetal Apgar score in the study group were significantly higher than those of the control group,and the incidence of cesarean section,dystocia and stillbirth were lower than those of the control group(P<0.05).Conclusions Comprehensive health care guidance and nursing for high-risk parturient women during pregnancy can help to improve the adverse psychological state of parturient women,reduce the incidence of complications during pregnancy and delivery,and have a positive effect on the prevention of neonatal complications,which is conducive to natural delivery of parturient women,reduce the occurrence of cesarean section and macrosomia,and is worthy of clinical promotion and application.
