摘　　要：目的探讨微小RNA-504(miR-504)、神经型钙黏附素(N-cadherin)mRNA、半乳糖凝集素9(Gal-9)mRNA与高度鳞状上皮内病变(HSIL)宫颈锥切术后高危型人乳头瘤病毒(HR-HPV)感染转阴关系及联合预测价值。方法将98例行宫颈锥切术HR-HPV阳性的HSIL患者,根据术后6个月HR-HPV感染是否转阴分为非转阴组27例、转阴组71例。比较两组患者一般资料和miR-504、N-cadherin mRNA、Gal-9 mRNA表达水平,采用多因素Logistic回归分析法分析HR-HPV感染转阴相关影响因素,采用受试者工作特征(ROC)曲线及曲线下面积(AUC)分析miR-504、N-cadherin mRNA、Gal-9 mRNA对HR-HPV感染转阴情况的预测价值。结果两组患者宫颈癌家族史、绝经情况比较,差异有统计学意义(P<0.05)。HR-HPV感染非转阴组患者miR-504表达水平低于转阴组,N-cadherin mRNA、Gal-9 mRNA表达水平高于转阴组(P<0.01);将宫颈癌家族史、绝经情况等基线资料控制后,miR-504、N-cadherin mRNA、Gal-9 mRNA表达水平仍与HR-HPV感染转阴情况相关(P<0.01);miR-504、N-cadherin mRNA、Gal-9 mRNA联合预测HR-HPV感染转阴的AUC最大。miR-504高水平表达患者的HR-HPV感染转阴率高于低水平表达患者,N-cadherin mRNA、Gal-9 mRNA高水平表达患者的HR-HPV感染转阴率低于低水平表达患者(P<0.01)。结论miR-504、N-cadherin mRNA、Gal-9 mRNA表达水平与HSIL宫颈锥切术后HR-HPV感染转阴有关,三者联合检测可作为预测HR-HPV感染转阴情况的有效方案。Objective To explore the relationship of microRNA-504(miR-504),neuro-cadherin(N-cadherin)mRNA and galectin 9(Gal-9)mRNA expression level with the negative conversion of high-risk human papillomavirus(HR-HPV)infection following cervical conization for high-grade squamous intraepithelial lesion(HSIL)and the value of combined prediction.Methods Ninety-eight HSIL patients with positive HR-HPV undergoing cervical conization were assigned to non-negative-conversion and negative-conversion group according to whether the HR-HPV infection turned negative in 6 months after operation.General data and the expression levels of miR-504,N-cadherin mRNA and Gal-9 mRNA were compared between two groups,related influencing factors of negative conversion of HR-HPV infection analyzed using multivariate Logistic analysis,and the predictive values of miR-504,N-cadherin mRNA and Gal-9 mRNA to negative conversion of HR-HPV infection done using receiver operating characteristic(ROC)curve and area under the curve(AUC).Results Intergroup differences in family history of cervical cancer and menopause were statistically significant(P<0.05).The expression level of miR-504 was lower and those of N-cadherin mRNA and Gal-9 mRNA were higher in non-negative-conversion than in negative-conversion group(P<0.01);after controlling basic data such as family history of cervical cancer and menopause,expression levels of miR-504,N-cadherin mRNA and Gal-9 mRNA were still related to the negative conversion of HR-HPV infection(P<0.01);the AUC of miR-504,N-cadherin mRNA and Gal-9 mRNA combination predicting the negative conversion of HR-HPV infection was the largest.The negative conversion rate of HR-HPV infection was higher in patients with high miR-504 expression than in ones with low,those were lower in patients with high N-cadherin mRNA and Gal-9 mRNA expression than in ones with low(P<0.01).Conclusion Expression levels of miR-504,N-cadherin mRNA and Gal-9 mRNA are related to the negative conversion of HR-HPV infection following cervical conization in HSIL
