哮喘合并OSAHS患儿外周血TNF-βIFN-γVEGF表达及其与气道阻力的相关性

摘  要:目的分析哮喘合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿外周血肿瘤坏死因子-β(TNF-β)、干扰素-γ(IFN-γ)、血管内皮生长因子(VEGF)表达及其与气道阻力的相关性。方法将119例哮喘合并OSAHS患儿设为哮喘合并OSAHS组,另选择同期收治的100例单纯哮喘患儿设为单纯哮喘组,检测全部患儿外周血TNF-β、IFN-γ及VEGF表达,并测量其气道阻力,分析TNF-β、IFN-γ、VEGF表达与气道阻力的相关性,并分析哮喘合并OSAHS患儿病情严重程度的影响因素。结果哮喘合并OSAHS组患儿外周血TNF-β、VEGF表达及呼吸道总阻力、中心气道阻力、外周弹性阻力均高于单纯哮喘组(P<0.01),IFN-γ表达低于单纯哮喘组(P<0.01)。Pearson相关分析显示,TNF-β、VEGF表达与呼吸道总阻力、中心气道阻力及外周弹性阻力均呈显著正相关(P<0.01),IFN-γ表达与呼吸道总阻力、中心气道阻力及外周弹性阻力均呈显著负相关(P<0.01)。多因素线性回归分析显示,体质量指数、TNF-β、IFN-γ、VEGF、呼吸道总阻力是影响哮喘合并OSAHS患儿病情严重程度的影响因素(P<0.05或0.01)。结论哮喘合并OSAHS患儿外周血TNF-β、VEGF表达及气道阻力升高,IFN-γ表达降低,三者表达与气道阻力密切相关,TNF-β、IFN-γ及VEGF可能通过影响气道阻力在哮喘合并OSAHS发生发展中发挥重要作用。Objective To analyze the expression levels of tumor necrosis factor-β(TNF-β),interferon-γ(IFN-γ)and vascular endothelial growth factor(VEGF)in peripheral blood and their correlation with airway resistance in children with asthma complicated with obstructive sleep apnea-hypopnea syndrome(OSAHS).Methods A total of 119 children with asthma complicated with the OSAHS were set as asthma+OSAHS and 100 ones with simple asthma treated in the corresponding period as asthma group,the expression levels of TNF-β,IFN-γ and VEGF in peripheral blood detected,airway resistances measured,and the correlations of TNF-β,IFN-γ and VEGF with airway resistance as well as factors influencing the severity of asthma+OSAHS analyzed.Results The TNF-β and VEGF expression level,total airway resistance(TAR),central airway resistance(CAR)and peripheral elastic resistance(PER)were higher(P<0.01)and IFN-γ was lower(P<0.01)in asthma+OSAHS than asthma group.Pearson correlation analysis showed that the TNF-β and VEGF expression were significantly positively correlated with the TAR,CAR and PER(P<0.01),and IFN-γ was negatively with the TAR,CAR and PER(P<0.01).Multivariate linear regression analysis showed that body mass index(BMI),TNF-β,IFN-γ,VEGF and TAR were factors influencing the severity of asthma with OSAHS(P<0.05).Conclusion The TNF-β and VEGF expression level in peripheral blood and airway resistance elevated in children with asthma+OSAHS,IFN-γ lowers,the expression of TNF-β,IFN-γ and VEGF are closely related with airway resistance,and the TNF-β,IFN-γ and VEGF plays important roles in the occurrence and development of asthma+OSAHS via influencing airway resistance.
