连续性肾脏替代治疗治疗脓毒血症的临床疗效观察

摘　　要：目的探讨脓毒血症患者应用连续性肾脏替代治疗(CRRT)的效果以及对血清炎性因子、淋巴细胞亚群的影响。方法选择2019年8月至2022年8月南阳市中心医院收治的92例脓毒血症患者,按随机数字表法分成对照组和观察组,各46例。对照组治疗方法为血液灌流,观察组在此基础上增加CRRT治疗,比较两组治疗疗效、血清炎性因子、淋巴细胞亚群、肾功能。结果观察组治疗有效率(95.65%)高于对照组(82.61%);治疗后两组降钙素原(PCT)、白细胞介素-6(IL-6)、C反应蛋白(CRP)值均低于治疗前,且观察组各指标均低于对照组;治疗后两组CD4^(+)、CD8^(+)值均高于治疗前,且观察组高于对照组;治疗后两组尿素氮(BUN)、血清肌酐(Scr)值均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论CRRT治疗脓毒血症疗效显著,能减轻血清炎性因子指标,提高淋巴细胞亚群水平和肾功能,值得临床应用。Objective To explore the effects of continuous renal replacement therapy(CRRT)on sepsis patients and its impact on serum inflammatory factors and lymphocyte subsets.Methods 92 septicopyemia patients treated from August 2019 to August 2022 in our hospital were selected.According to random number table,patients were assigned to control group(n=46,blood perfusion)and observation group(n=46,blood perfusion and CRRT).The clinical effects,serum inflammatory factors,lymphocyte subpopulation and renal function were compared.Results The total effective rate of observation group was 95.65%,which was higher than that of control group 82.61%(P<0.05).After treatment,the values of procalcitonin(PCT),interleukin-6(IL-6)and C-reactive protein(CRP)in the observation group were lower than those in the control group(P<0.05),and those in the two groups were lower than those before treatment(P<0.05).After treatment,CD4^(+) and CD8^(+) values in observation group were higher than those in control group(P<0.05),and those in both groups were higher than those before treatment(P<0.05).After treatment,the values of urea nitrogen(BUN)and serum creatinine(Scr)in the observation group were lower than those in the control group(P<0.05),and those in the two groups were lower than those before treatment(P<0.05).Conclusion CRRT can reduce the serum inflammatory factors,improve the lymphocyte subpopulation and renal function for septicopyemia patients.It is worthy of clinical application.
