腹腔镜髂耻韧带悬吊术治疗女性盆腔器官脱垂的疗效分析

摘　　要：目的探究腹腔镜髂耻韧带悬吊术治疗女性盆腔器官脱垂(POP)的临床效果。方法选择2020年12月至2022年12月在濮阳市安阳地区医院治疗的102例女性盆腔器官脱垂患者,应用随机数表法将其分为对照组(骶骨固定术)及观察组(髂耻韧带悬吊术),各51例。比较两组临床指标、治疗有效率、尿动力学指标、盆腔器官脱垂情况、并发症发生率。结果观察组手术时间、术中出血量、住院时间短于对照组,差异有统计学意义(P<0.05)。对照组治疗有效率低于观察组,差异有统计学意义(P<0.05)。观察组尿流动力学指标优于对照组,差异有统计学意义(P<0.05)。观察组盆腔器官脱垂情况(POP-Q)指示点(C、D)小于对照组,差异有统计学意义(P<0.05)。对照组并发症发生率高于观察组,差异有统计学意义(P<0.05)。结论对女性盆腔器官脱垂患者采用腹腔镜髂耻韧带悬吊术治疗,疗效显著,可改善患者盆腔器官脱垂状态,促进膀胱功能恢复,安全性较高。Objective To investigate the clinical effect of laparoscopic iliopectineal ligament suspension in the treatment of female pelvic organ prolapse(POP).Methods A total of 102 female patients with pelvic organ prolapse treated in our hospital from December 2020 to December 2022 were selected and divided into control group(sacral fixation)and observation group(iliopubic ligament suspension)by random number table method,with 51 cases in each group.Results The operation time,intraoperative blood loss and hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The treatment effective rate of control group was lower than that of observation group,the difference was statistically significant(P<0.05).The urinary flow dynamics indexes of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).The improvement of pelvic organ prolapse(POP-Q)indicator points(C and D)of the observation group were smaller than those of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in control group was higher than that in observation group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic iliopectineal ligament suspension is effective in the treatment of female patients with pelvic organ prolapse,which can improve the state of pelvic organ prolapse and promote the recovery of bladder function with high safety.
