腹腔镜肾囊肿去顶术与穿刺硬化术治疗多囊肾疗效的Meta分析

摘　　要：目的评价腹腔镜肾囊肿去顶术与穿刺硬化术治疗多囊肾的疗效。方法计算机检索PubMed、EMbase、The Cochrane Library、Web of Science、CBM、WanFang Data、CNKI和维普数据库,搜集关于腹腔镜肾囊肿去顶术与肾囊肿穿刺硬化术治疗多囊肾疗效的研究,检索时间从建库至2018年8月。由两位评价员独立筛选文献,提取资料并评价纳入研究的偏倚风险等,采用STa Ta 12.0软件进行Meta分析。结果最终纳入8个研究,包括385例患者。Meta分析结果显示,腹腔镜肾囊肿去顶术与肾囊肿穿刺硬化术对多囊肾患者疼痛和高血压的改善,差异无统计学意义(P>0.05),但两种方法对疼痛和高血压都有明显改善。结论腹腔镜肾囊肿去顶术与肾囊肿穿刺硬化术对多囊肾患者疼痛和高血压的改善都有积极作用,但二者的差异无优劣势表现。鉴于纳入研究质量及数量有限,上述结论尚需更多及高质量的研究予以验证。Objective To Systematically review the efficacy of laparoscopic cyst decortication and aspiration and sclerotherapy in the treatment of autosomal dominant polycystic kidney disease(ADPKD).Methods We searched databases including PubMed、EMbase、The Cochrane Library、Web of Science、CBM、WanFang Data、CNKI and WIP Data to collect relevant clinical studies comparing the efficacy of laparoscopic cyst decortication and aspiration and sclerotherapy for ADPKD from inception to August,2018.Two reviewers independently screened literature,extracted data and assessed the risk bias of included studies.Then,STaTa 12.0 software was used for meta-analysis.Results A total of eight case-series studies involving 385 patients were included.The results of the meta-analysis showed that:There was no significant difference between two methods in the efficacy of hypertension and pain.But both methods can significantly improve the symptoms of hypertension and pain.Conclusion Available evidence suggests that laparoscopic cyst decortication and aspiration and sclerotherapy have positive effects on pain and hypertension in patients with polycystic kidney disease,but there has no obviously difference between two methods.Due to the limited quality and quantity of the included studies,more high quality studies are needed to verify the above conclusion.
