基于异病同治理论探讨滋阴解毒法辨证治疗靶向药物所致皮肤黏膜不良反应

摘　　要：从异病同治的理论出发,通过对酪氨酸激酶抑制剂(Epithelial growth factor receptor-tyrosine kinase inhibitors,EGFR-TKIs)所致皮肤及黏膜相关不良反应的西医发生机制及中医的病因病机进行分析,认为滋阴解毒法为肺癌的中医主要治法,与靶向治疗后所致的皮疹、腹泻及口腔溃疡的病机特点高度一致,三者虽然临床表现不一,但证候相同,皆为阴虚毒热所致,为中医异病同治方法提供了物质与理论基础。滋阴解毒治疗大法应贯穿肺癌的发生发展及靶向治疗的全过程,这可能为中医“减毒增效”,延缓耐药甚至减少复发提供治疗新思路。Starting from the theory of treating different diseases with the same method,this paper analyzed the mechanism of skin and mucosa-related adverse reactions caused by epithelial growth factor receptor-tyrosine kinase in⁃hibitors(EGFR TKIs)in western medicine and the etiology and pathogenesis in traditional Chinese medicine(TCM),It pro⁃posed that nourishing yin and detoxification as the main TCM treatment for lung cancer is highly consistent with the patho⁃gensis of skin rashes,diarrhea and oral ulcer due to targeted therapy.Although the clinical manifestations of the three are different,but the syndrome is the same,which is yin deficiency and toxin heat.This provides the material and theoretical basis for treating different diseases with the same method in TCM.Nourishing yin and detoxification should be used throughout the whole process of lung cancer and targeted therapy,which may provide a new treatment idea for"reducing toxicity and enhancing efficacy",delaying drug resistance and even reducing recurrence in TCM.
