摘 要:目的探讨肠内营养(EN)治疗对白族老年多器官功能不全(MODSE)合并糖尿病(DM)患者营养及免疫指标的影响。方法120例大理白族MODSE合并DM患者,随机分为实验组和对照组,每组60例。实验组患者给予DM专用的肠内营养混悬液(TPF-DM)+常规饮食方案治疗,对照组给予常规饮食方案治疗。比较两组患者能量及蛋白质摄入量,治疗前后营养指标[白蛋白(ALB)、总蛋白(TP)、血红蛋白(Hb)]、免疫指标[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]、C反应蛋白(CRP)、空腹血糖(FPG)水平。结果两组患者能量及蛋白质摄入量比较差异无统计学意义(P>0.05)。治疗10 d后,两组患者ALB、TP、Hb水平均高于本组治疗前,且实验组患者ALB(40.56±2.79)g/L、TP(70.37±7.20)g/L、Hb(126.00±30.25)g/L明显高于对照组的(36.17±2.79)、(64.80±7.27)、(110.00±28.25)g/L,差异有统计学意义(P<0.05)。治疗10 d后,实验组患者的IgA、IgG、IgM水平高于本组治疗前,CRP、FPG低于本组治疗前,且实验组患者的IgA(2.95±0.66)g/L、IgG(12.19±5.15)g/L、IgM(1.60±0.42)g/L明显高于对照组的(2.42±0.64)、(9.57±5.80)、(1.36±0.35)g/L,CRP(7.20±2.92)mg/L、FPG(6.01±1.12)mmol/L明显低于对照组的(14.10±5.97)mg/L、(8.40±1.04)mmol/L,差异具有统计学意义(P<0.05)。结论对白族MODSE合并DM患者给予肠内营养治疗可改善营养状况及免疫功能,降低CRP、FPG水平,疗效及预后均会得到明显改善。Objective To discuss the effects of enteral nutrition(EN)therapy on nutritional and immune indexes in patients of Bai nationality with multiple organ dysfunction syndrome in the elderly(MODSE)and diabetes mellitus(DM).Methods A total of 120 patients with MODSE and DM of Bai nationality were randomly divided into the experimental group and the control group,with 60 cases in each group.The experimental group was given DM specific enteral nutrition suspension(TPF-DM)+conventional diet program,and the control group was given conventional diet program.Both groups were compared in terms of energy and protein intake,nutritional indexes[albumin(ALB),total protein(TP),hemoglobin(Hb)],immune indexes[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)],C-reactive protein(CRP),fasting blood glucose(FPG)levels before and after treatment.Results There was no statistically significant difference in energy and protein intake between the two groups(P>0.05).After 10 d of treatment,the levels of ALB,TP and Hb in both groups were higher than those before treatment in this group;the experimental group had ALB of(40.56±2.79)g/L,TP of(70.37±7.20)g/L and Hb of(126.00±30.25)g/L,which were significantly higher than those of(36.17±2.79),(64.80±7.27)and(110.00±28.25)g/L in the control group;the differences were statistically significant(P<0.05).After 10 d of treatment,the levels of IgA,IgG and IgM in the experimental group were higher than those before treatment in this group,while CRP and FPG were lower than those before treatment in this group;the experimental group had IgA of(2.95±0.66)g/L,IgG of(12.19±5.15)g/L and IgM of(1.60±0.42)g/L,which were significantly higher than those of(2.42±0.64),(9.57±5.80)and(1.36±0.35)g/L in the control group;CRP of(7.20±2.92)mg/L and FPG of(6.01±1.12)mmol/L in the experimental group were significantly lower than those of(14.10±5.97)mg/L and(8.40±1.04)mmol/L in the control group;the differences were statistically significant(P<0.05).Conclusion Enteral nutrition therapy can imp
