双极人工股骨头置换术与全髋关节置换术治疗老年股骨颈骨折的疗效比较

摘　　要：目的比较老年股骨颈骨折患者采用双极人工股骨头置换术和全髋关节置换术治疗的临床效果。方法回顾性分析40例老年股骨颈骨折患者的临床资料,根据患者入院顺序分为对照组和研究组,每组20例。对照组患者接受双极人工股骨头置换术治疗,研究组患者接受全髋关节置换术治疗。对比两组患者手术时间、手术期间出血量、术后引流量、住院时间、术后并发症发生情况和术后髋关节功能评分。结果研究组患者手术时间(91.4±19.7)min、住院时间(23.4±4.8)d明显长于对照组的(47.1±9.9)min、(14.1±5.3)d,手术期间出血量(336.0±193.7)ml、术后引流量(119.2±84.4)ml均明显多于对照组的(126.5±75.4)、(74.2±47.2)ml,术后髋关节功能评分(84.2±3.6)分明显高于对照组的(71.0±3.1)分,差异有统计学意义(P<0.05)。两组患者术后并发症发生率对比,差异无统计学意义(P>0.05)。结论临床对老年股骨颈骨折患者采取双极人工股骨头置换术治疗,可以帮助患者缩短手术时间及住院时间,减少患者术中出血量和术后引流量;给予全髋关节置换术,患者可以获得良好的髋关节功能恢复效果,两种手术治疗方式均存在较高的安全性,需要依照患者具体情况开展对应治疗,保证手术治疗效果。Objective To compare the clinical effect of bipolar artificial femoral head replacement and total hip arthroplasty in the treatment of femoral neck fracture in the elderly.Methods The clinical data of 40 elderly patients with femoral neck fracture were retrospectively analyzed,and they were divided into the control group and the study group according to the order of admission,with 20 cases in each group.The control group received bipolar artificial femoral head replacement,and the study group received total hip arthroplasty.Both groups were compared in terms of operative time,intraoperative blood loss,postoperative drainage volume,hospital stay,occurrence of postoperative complications,and postoperative hip joint function score.Results The operative time of(91.4±19.7)min and hospital stay of(23.4±4.8)d in the study group were significantly longer than those of(47.1±9.9)min and(14.1±5.3)d in the control group;the intraoperative blood loss of(336.0±193.7)ml and postoperative drainage volume of(119.2±84.4)ml in the study group were significantly higher than those of(126.5±75.4)and(74.2±47.2)ml in the control group;the hip function score of(84.2±3.6)points in the study group was significantly higher than that of(71.0±3.1)points in the control group;the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Bipolar artificial femoral head replacement for elderly patients with femoral neck fracture can help patients shorten the operative time and hospital stay,reduce the intraoperative blood loss and postoperative drainage volume.After total hip arthroplasty,the patient can obtain good hip functional recovery effect.Both surgical treatment methods have high safety,and corresponding treatment should be carried out according to the specific situation of the patient to ensure the surgical treatment effect.
