寰枢关节紊乱综合征患者症状与X线表现的相关性研究

摘　　要：目的分析寰枢关节紊乱综合征的症状与其X线表现的相关性。方法回顾性分析40例寰枢关节紊乱综合征患者的临床资料,分析患者的Northwick Park颈痛量表中文版(NPQ)、颈性眩晕症状与功能评估量表(ESCV)评分与X线检查下双侧齿突侧块间距差值(VBLADI)的相关性。结果通过软件绘制散点图,以VBLADI为X轴,NPQ评分为Y轴,通过观测数据大致符合线性相关关系。通过软件绘制散点图,以VBLADI为X轴,ESCV评分为Y轴,通过观测数据大致符合线性相关关系。Spearman秩相关检验显示,NPQ评分与VBLADI存在紧密的正相关关系(r=0.406,P=0.009<0.01),ESCV评分与VBLADI存在紧密的负相关关系(r=-0.611,P=0.000<0.01)。结论NPQ、ESCV评分均与VBLADI有密切的关系,患者相关症状表现越重,其关节紊乱的情况越严重,在临床上可通过影像学检查来辅助诊断及判断寰枢关节紊乱综合征患者的病情,避免误诊、漏诊。Objective To analyze the correlation between symptoms and X-ray findings in patients with atlanto-axial joint disorder syndrome.Methods The clinical data of 40 patients with atlanto-axial joint disorder syndrome were retrospectively analyzed,and the correlation of the Northwick Park Neck Pain Questionnaire(NPQ),the evaluation scale for cervical vertigo(ESCV)scores with variance of bilateral atlanto-dental interval(VBLADI)by X-ray examination was analyzed.Results The scatter-plot plot was drawn by software,with VBLADI as the X-axis and NPQ score as the Y-axis,and the linear correlation was roughly consistent with the observed data.The scatter plots were drawn by software,with VBLADI as the X-axis and ESCV score as the Y-axis,and the linear correlation was roughly consistent with the observed data.Spearman rank correlation test showed a close positive correlation between NPQ score and VBLADI(r=0.406,P=0.009<0.01),and a close negative correlation between ESCV score and VBLADI(r=-0.611,P=0.000<0.01).Conclusion Both NPQ and ESCV scores are closely related to VBLADI.The more severe the symptoms,the joint disorder would be more serious.Patients with atlanto-axial joint disorder syndrome can be diagnosed and judged by imaging examination to avoid misdiagnosis and missed diagnosis.
