典型阴茎折断的临床诊断探讨与早期手术疗效分析

摘　　要：目的 探讨具有典型临床表现的阴茎折断的诊断要点与早期手术疗效。方法 回顾性分析福鼎市医院于2014年5月至2022年8月间收治的21例阴茎折断患者的临床诊治及手术治疗经过,术前均未采取B超等辅助诊断。患者平均年龄41.5岁(25~58岁),平均病程9 h(0.5~18 h),均因粗暴性交所致,均无排尿困难及尿道口出血。所有患者均在腰麻下行急诊手术探查及阴茎海绵体白膜修补术,其中18例采用冠状沟近侧环行切口,3例采用局部切口;术后加压包扎、留置导尿。结果 21例患者均为单侧海绵体白膜破裂,其中左侧12例、右侧9例;裂口均为横行,长度0.3~1.2 cm,位于阴茎海绵体近端2/3处;血肿局限于阴茎皮下;无伴尿道及尿道海绵体损伤。所有患者术后随访7~15个月,未出现阴茎畸形、痛性硬结、勃起功能障碍及性生活质量下降等并发症。结论 典型的阴茎折断根据症状及体征即可明确临床诊断,急诊手术探查并修复可获得良好的治疗效果。Objective To investigate the clinical diagnosis and early operative effect of penile fracture with typical clinical manifestations.Methods The clinical diagnosis and surgical treatment of 21 patients with typical penile fracture admitted to Fuding Hospital from May 2014 to August 2022 were retrospectively analyzed.None of them received B-ultrasonography and other auxiliary diagnosis before operation.The average age of the patients was 41.5 years old(25-58 years old),and the average course of the disease was 9 hours(0.5-18 hours).All patients were caused by rough intercourse,no dysuria or urethral opening bleeding was reported.All patients underwent emergency surgical exploration and repair of tunica alba of the cavernosum of the penis under lumbar anesthesia.Among them,18 patients underwent proximal coronary groove circumferential incision,and 3 patients underwent local incision.Postoperative pressure bandaging and indwelling catheterization.Results All 21 patients had unilateral rupture of albuginea of cavernous body,including 12 patients on the left side and 9 patients on the right side.The clefts are all transverse,measuring 0.3-1.2 cm in length and located at the proximal 2/3 of the corpora cavernosa.The hematoma was confined to the subcutaneous area of the penis.There was no associated urethral and urethral cavernous injury.All patients were followed for 7-15 months postoperatively and showed no complications such as penile deformity,painful induration,erectile disorder and decreased quality of sexual life.Conclusion Typical penile fracture can be clinically diagnosed according to symptoms and signs,and good therapeutic effect can be obtained by emergency surgical exploration and repair.
