甲硝唑联合雌二醇凝胶对老年混合性阴道炎患者阴道微生态和机体免疫的影响

摘　　要：目的 分析甲硝唑联合雌二醇凝胶对老年混合性阴道炎患者阴道微生态和机体免疫的影响。方法 选取2019年3月至2022年3月衢州市妇幼保健院收治的76例老年混合性阴道炎患者作为研究对象,随机分为对照组与研究组,各38例。对照组给予甲硝唑治疗,研究组给予甲硝唑联合雌二醇凝胶治疗,均治疗2周。比较两组临床疗效、症状消失时间、阴道微生态(pH值、菌群密集度、菌群多样性、优势菌群)、机体免疫[血清白介素-2(IL-2)、白介素-10(IL-10)、分泌性免疫球蛋白A(SIgA)、免疫球蛋白G(IgG)水平]、不良反应及病情复发状况。结果 治疗2周后,研究组有效率为97.37%,高于对照组的81.58%(P<0.05);研究组阴道瘙痒、阴道灼痛、分泌物减少、阴道黏膜肿胀等症状缓解或消失时间均短于对照组(P<0.05);研究组阴道pH值、菌群密集度、菌群多样性及优势菌群数均优于对照组(P<0.05);研究组血清IL-2、IL-10水平低于对照组,SIgA、IgG水平高于对照组(P<0.05);研究组不良反应发生率为7.89%,低于对照组的10.53%,但两组间比较无显著差异(P>0.05);停药4周内,研究组病情复发率为10.81%,显著低于对照组的35.48%(P<0.05)。结论 甲硝唑联合雌二醇凝胶治疗老年混合性阴道炎患者,可提升疗效,有效改善患者阴道微生态,增强机体免疫力,降低疾病复发率,安全性好。Objective To analyze the effects of metronidazole combined with estradiol gel on vaginal microecology and immunity in elderly patients with mixed vaginitis.Methods A total of 76 elderly patients with mixed vaginitis admitted to Quzhou Maternal and Child Health Hospital were enrolled as the research objects between March 2019 and March 2022.They were randomly divided into control group and study group,38 cases each.The control group was treated with metronidazole,while study group was treated with metronidazole combined with estradiol gel.All patients were treated for 2 weeks.The clinical curative effect,disappearance time of symptoms,vaginal microecology(pH,flora intensity,flora diversity,dominant floras),immunity[interleukin-2(IL-2),interleukin-10(IL-10),secretory immunoglobulin A(SIgA),immunoglobulin G(IgG)levels],adverse reactions and disease recurrence were compared between the two groups.Results After 2 weeks of treatment,response rate of study group was higher than that of control group(97.37%vs.81.58%,P<0.05).The remission or disappearance time of symptoms(vaginal itching,vaginal burning pain,reduction of secretion,vaginal mucosa swelling)in study group was shorter than that in control group(P<0.05).Vaginal pH,flora intensity,flora diversity and number of dominant floras were better than those in control group(P<0.05).The levels of serum IL-2 and IL-10 in study group were lower than those in control group,while levels of SIgA and IgG were higher than those in control group(P<0.05).The incidence of adverse reactions in study group was lower than that in control group(7.89%vs.10.53%),but there was no significant difference between the two groups(P>0.05).Within 4 weeks after drug withdrawal,disease recurrence rate in study group was significantly lower than that in control group(10.81%vs.35.48%,P<0.05).Conclusions Metronidazole combined with estradiol gel can improve curative effect,effectively improve vaginal microecology,enhance immunity and reduce disease recurrence rate in elderly patients with mixed vagi
