摘 要:目的 通过分析实验室的精液参数分布,为监测实验室精液分析准确性提供新的质量控制评估方法。方法 回顾性分析2018年—2020年广东省生殖医院人类精子库捐精志愿者的精液分析结果,统计精液量、精子浓度、精子活力及精子正常形态率等精液参数的百分位数,并与《世界卫生组织人类精液检查与处理实验室手册》(第5版)(以下简称WHO 5^(th))公布的源自性伴侣在停止使用避孕措施后12个月内怀孕的男性的精液参数值分布进行比较分析。结果 共纳入4 761例捐精志愿者,年龄22~45岁,中位年龄25岁;精液量和前向运动精子百分率的各个百分位数与WHO 5^(th)分布比较一致,说明本实验室精液量和前向运动精子百分率分析结果的准确度较高;精子浓度和精子正常形态率的各个百分位数与WHO 5^(th)相比较,在第75个百分位数之后一致性较差,说明本实验室在精子浓度或精子正常形态率较高时,精子浓度及精子正常形态率的分析结果存在较大偏差,需要进一步改进。结论 精液参数分布能在一定程度上监测实验室内精液分析的准确性,并通过分析数据的准确性来寻找问题的根源并进行改进。Objective To provide a new quality control evaluation method for monitoring the accuracy of semen analysis by analyzing the distribution of semen parameters in the laboratory.Methods The results of semen analysis of human sperm donors in Human Sperm Bank of Guangdong Provincial Fertility Hospital from 2018 to 2020 were retrospectively analyzed.The percentiles of semen parameters such as semen volume,sperm concentration,sperm motility and normal morphology rate were calculated.The results were compared with the distribution of values for semen parameters from men whose partners became pregnant within 12 months of discontinuing contraceptive use published in World Health Organization(WHO)laboratory manual for the examination and processing of human semen(5 th Edition)(hereinafter referred to as WHO 5 th).Results A total of 4,761 sperm donors were included,aged from 22 to 45 years old(median age was 25).The percentiles of semen volume and progressive motility were consistent with WHO 5 th distribution,which indicated that the accuracy of semen volume and progressive motility analysis in our laboratory was high.Compared with WHO 5 th,the consistency of each percentile of sperm concentration and normal morphology rate were poor after the 75th percentile,which indicated that there was a large deviation in the analysis results of sperm concentration and normal morphology rate when the sperm concentration or normal forms were high in our laboratory,which needs further improvement.Conclusions The distribution of semen parameter values can monitor the accuracy of semen analysis in the laboratory to a certain extent,and find the root of the problem and improve it by analyzing the accuracy of data。
