探究腹腔镜下保留睾丸动脉的精索内静脉高位结扎术对VC患者性功能、精液质量及性激素水平的影响

摘　　要：目的 分析腹腔镜下保留睾丸动脉的精索内静脉高位结扎术对精索静脉曲张(VC)患者性功能、精液质量及性激素水平的影响。方法 选取2017年1月至2020年1月在恩施土家族苗族自治州中心医院就诊的90例VC患者作为研究对象,按照随机数字表法分为对照组和观察组,各45例。均给予腹腔镜下精索内静脉高位结扎术,对照组不保留睾丸动脉,观察组保留。观察并比较两组手术情况、精液质量及性功能水平。结果 观察组手术时间显著长于对照组(P<0.05),术中出血量、住院时间及住院费用均无显著差异(P>0.05);术后两组性功能评分、精液质量指标水平、睾酮水平较术前升高,卵泡刺激素(FSH)和黄体生成素(LH)水平较术前降低,且观察组上升或降低幅度均高于对照组(P<0.05)。结论 VC患者行保留睾丸动脉的精索内静脉高位结扎术能有效改善患者精液质量及性功能、性激素水平。Objective To analyze the influence of laparoscopic high ligation of spermatic vein with preserved testicular artery on sexual function,semen quality and sex hormones of patients with varicocele(VC).Methods A total of 90 patients with VC who were treated in the Central Hospital of Enshi Tujia and Miao Autonomous Prefecture from January 2017 to January 2020 were selected as the research subjects.According to the random number table method,the patients were divided into control group and observation group with 45 cases in each group.All patients were treated with laparoscopic high ligation of the spermatic vein.The testicular artery was not preserved in the control group and preserved in the observation group.Surgical conditions,semen quality and sexual function were compared between the two groups.Results The operation time of observation group was significantly longer than that of control group(P<0.05),but there was no significant difference in blood loss,hospital stay or hospitalization expenses(P>0.05).After operation,the sexual function scores,semen quality and testosterone level were increased,while follicle stimulating hormone(FSH)and luteinizing hormone(LH)levels were reduced in the two groups.The increases or decreases in observation group were greater than those in control group(P<0.05).Conclusions High ligation of spermatic vein with preserved testicular artery can effectively improve semen quality,sexual function and sex hormone levels in patients with VC.
