摘　　要：目的:采用静息态fMRI观察产后精神病(PP)患者大脑半球间基于体素的镜像同伦功能连接(VMHC)变化特点。方法:采集20例PP患者(PP组)及22例健康产妇(对照组)静息态fMRI数据,以VMHC分析技术观察2组大脑半球间功能连接变化,提取2组差异脑区的VMHC值,并与简明精神病评定量表(BPRS)评分行相关性分析。结果:与对照组相比,PP组大脑半球间岛叶、海马、中央前回的VMHC值均降低(GRF校正,均P<0.05)。PP组海马、岛叶的VMHC值均与BPRS评分呈负相关(r=-0.57,P=0.01;r=-0.45,P=0.04)。结论:静息态下PP患者与情绪、认知相关的脑区存在大脑半球间功能连接的异常活动,涉及默认网络、突显网络、记忆网络及感觉运动网络的异常变化,这些发现有助于深入理解PP患者脑神经功能改变的特点。Objective:To explore the changes in voxel-mirrored homotopic connectivity(VMHC)in postpartum psychosis(PP)patients using resting-state fMRI(rs-fMRI).Methods:The rs-fMRI data of 20 PP patients(the PP group)and 22 healthy postpartum women(the control group)were collected.The VMHC approach was used to observe functional connectivity changes between the cerebral hemispheres of the two groups.The VMHC values were extracted from different regions in the two groups,and the correlation with the Brief Psychiatric Rating Scale(BPRS)score was analyzed.Results:Compared with the control group,the PP group showed decreased VMHC values in the hippocampus and precentral gyrus(all P<0.05).The VMHC values of the hippocampus and the insula were negatively correlated with BPRS score in the PP group(r=-0.57,P=0.01;r=-0.45,P=0.04).Conclusions:Abnormal cerebral hemisphere functional connectivity changes exist in the emotion-and cognition-related brain regions in PP patients,involving disorders in the default mode network,salience network,memory network and sensorimotor network.These findings are helpful to deeply understand the characteristics of brain neural function changes in patients with PP.
