青少年肥胖合并良性黑棘皮病与代谢综合征研究

摘　　要：目的分析青少年肥胖合并良性黑棘皮病患者与单纯肥胖患者在基础生理指标参数以及糖代谢指标参数方面的差异,提升青少年代谢综合征认识。方法于2021年1月-2022年12月期间在天津市泰达医院就诊的青少年肥胖患者中抽选76例进行分组对照研究,38例单纯肥胖患者作为对照组,38例合并良性黑棘皮病的肥胖患者作为实验组,通过分析两组患者基础生理指标参数以及糖代谢指标参数明确黑棘皮病在青少年代谢综合征诊断治疗方面的临床价值。结果相较于对照组,实验组患者体重指数、腰围、血压、甘油三酯水平、空腹胰岛素浓度、OTGG参数以及胰岛素抵抗指数更高。两组患者总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇以及空腹血糖水平差异较小。结论青少年肥胖患者是否合并良性黑棘皮病可用于判断其糖代谢情况以及当前Ⅱ型糖尿病发展风险。Objective To analyze the differences of basic physiological parameters and glucose metabolism parameters between adolescent obese children with benign acanthosis nigricans and obese patients,so as to improve the understanding of adolescent metabolic syndrome.Methods From January 2021 to December 2022,76 adolescent obese patients admitted to the hospital were selected for a grouped control study,38 obese patients with simple obesity as the control group and 38 obese patients with benign acanthosis nigricans as the experimental group.The clinical value of acanthosis nigricans in the diagnosis and treatment of adolescent metabolic syndrome was determined by analyzing the basic physiological parameters and glucose metabolism parameters of the two groups of patients.Results Compared with the control group,the body mass index,waist circumference,blood pressure,triglyceride level,fasting insulin concentration,OTGG parameters and insulin resistance index were higher in the experimental group.There were little differences in total cholesterol,HDL cholesterol,LDL cholesterol,and fasting blood glucose levels between the two groups.Conclusion Whether adolescent obese patients have benign acanthosis nigricans can be used to determine their glucose metabolism and the current risk of developing type 2 diabetes.
