S-100吸收止血绫在重度创伤性颅脑损伤术中的应用

摘　　要：目的分析S-100吸收止血绫在重度创伤性颅脑损伤术中的应用效果。方法回顾性分析2022年8月-2023年4月就诊于甘肃省人民医院神经外科开颅手术的100例重度颅脑损伤患者临床资料,根据术中采取不同的止血方式分为对照组和研究组,每组各50例。对照组采用常规明胶海绵及脑棉片止血,研究组患者采用S-100吸收止血进行止血处理,对比两组患者术区止血有效率、术中出血量、手术时间及术后拔管时间,观察两组患者术前及术后24h凝血功能指标变化。结果研究组患者术区止血有效率为96%,高于对照组的84%,且研究组术中出血量、手术时间及术后拔管时间均少于对照组,P<0.05;术后24h,两组患者凝血活酶时间(APTT)、凝血酶原时间(PT)及D-二聚体(DD)均有所增加,纤维蛋白原(FIB)水平均有所降低,但研究组APTT、PT及DD均低于对照组,FIB高于对照组患者,P<0.05。结论S-100吸收止血绫能提高重度创伤性颅脑损伤术后止血效果,减少术中出血量及手术时间,改善患者术后凝血功能,促进患者预后。Objective To analyze the application effect of S-100 absorbable hemostatic gauze in severe traumatic brain injury surgery.Methods The clinical data of 100 patients with severe craniocerebral injury who received Craniotomy in Neurosurgery department of Gansu Provincial People's Hospital from August 2022 to April 2023 were retrospectively analyzed.According to different hemostasis methods during the operation,they were divided into control group and study group with 50 patients in each group.The control group used conventional gelatin sponge and brain cotton pads for hemostasis,while the study group patients used S-100 absorption hemostatic gauze for hemostasis treatment.The two groups of patients were compared in terms of effective hemostasis efficiency,intraoperative bleeding volume,surgical time,and postoperative extubation time.The changes in coagulation function indicators at before and 24 hours after surgery were observed.Results The effective rate of hemostasis in the study group was 96%,higher than 84%in the control group,and the intraoperative bleeding volume,surgical time,and postoperative extubation time in the study group were all lower than those in the control group(P<0.05);at 24 hours after operation,the thromboplastin time(APTT),thrombin time(PT)and D-dimer(DD)of patients in both groups increased,and the level of fibrinogen(FIB)decreased.However,APTT,PT and DD in the study group were lower than those in the control group,and FIB was higher than those in the control group,P<0.05.Conclusion S-100 absorbable hemostatic gauze can improve the postoperative hemostatic effect of severe traumatic brain injury,reduce intraoperative bleeding and surgical time,improve postoperative coagulation function,and promote patient prognosis.
