摘　　要：目的分析维持性血液透析患者其Cuff导管应用不同抗凝剂的封存效果。方法此次研究的时间段为2021年10月-2022年12月,选取该期间到江苏省南通市通州区中医院接受Cuff导管的维持性血液透析患者作为研究对象,共30例,以电脑抽号法作为分组方式,将其分为两组,即研究组(n=15)和常规组(n=15),对研究组的患者应用枸橼酸钠进行封管处理,对常规组的患者应用肝素钠进行封管。对比两组患者封管效果的差异性。结果研究组患者的CRIs发生次数、导管内抽吸血栓次数、导管功能不良发生次数低于常规组(P<0.05);研究组患者的尿激酶封管次数高于常规组(P<0.05);研究组患者干预后的PT、APTT指标高于常规组(P<0.05);研究组患者的不良反应发生率低于常规组(P<0.05)。研究组患者的Ca^(2+)、Mg^(2+)、AST、ALT、血红蛋白以及血小板计数等指标与常规组比较,差异不存在统计学意义(P>0.05)。结论对Cuff导管的维持性血液透析患者而言,枸橼酸钠抗凝剂可以有效减少不良反应的发生,但是存在发生导管内血栓的风险,因此临床需要根据患者的实际情况选择适宜的抗凝剂。Objective To analyze the sealing effect of Cuff catheter with different Anticoagulant in maintenance hemodialysis patients.Method This study began in October 2021 and ended in December 2022.A total of 30 maintenance hemodialysis patients who received Cuff catheters at our hospital during this period were selected as the study subjects.They were divided into two groups using computer number selection method.The study group(n=15)and the conventional group(n=15)were named as the two groups,and the patients in the study group were sealed with sodium citrate,patients in the conventional group were sealed with heparin sodium.Compare the differences in sealing effects between two groups of patients.Result The incidence of CRIs,the number of intravascular emboli drawn,incidence of catheter dysfunction in the study group was even lower than that in the conventional group(P<0.05);the number of Urokinase tube sealing in the study group was higher than that in the conventional group(P<0.05);the PT,APTT of the study group patients were higher than those of the conventional group(P<0.05);the incidence of adverse reactions in the study group was even lower than that in the conventional group(P<0.05).There was no statistically significant difference(P>0.05)between the study group and the conventional group in terms of Ca^(2+),Mg^(2+),AST,ALT,hemoglobin,and platelet count.Conclusion For maintenance hemodialysis patients with Cuff catheter,sodium citrate Anticoagulant can effectively reduce the occurrence of adverse reactions,but there is a risk of thrombosis in the catheter,so clinical needs to select appropriate Anticoagulant according to the actual situation of patients.
