腹腔镜下隐睾下降固定术治疗小儿高位隐睾的临床研究

摘　　要：目的探讨腹腔镜下隐睾下降固定术治疗小儿高位隐睾的临床效果。方法选取广东省英德市人民医院2019年8月-2021年12月收治的40例高位隐睾患儿为研究对象,按照患儿的治疗方式分为两组,每组各20例。其中对照组患儿采取开放式手术治疗,研究组采取腹腔镜下隐睾下降术治疗。比较两组患儿治疗疗效、手术一般情况、疼痛程度、并发症及随访情况。结果研究组治疗总有效率与对照组比较,差异不显著(P>0.05)。研究组术中出血量、住院时间、切口长度指标水平均低于对照组,对比差异显著(P<0.05)。术后6h、术后12h、术后1d研究组患儿的疼痛程度评分低于对照组,对比差异显著(P<0.05)。研究组并发症发生率5.00%(1/20),低于对照组并发症发生率,但对比差异不显著(P>0.05)。随访结果显示,两组患儿睾丸萎缩、回缩对比差异不显著(P>0.05)。结论腹腔镜下隐睾下降固定术治疗小儿高位隐睾的临床效果满意,可控制创伤并缩短康复时间,降低并发症的发生率和疼痛程度,并且还能够保障患儿获得理想的远期疗效。Objective To explore the clinical effect of laparoscopic epididymis descent fixation surgery for the treatment of high cryptorchidism in children.Method Forty children with high cryptorchidism admitted to our hospital from August 2019 to December 2021 were selected as the research subjects,and were divided into groups according to their treatment methods,with 20 cases each.The control group was treated with open surgery,while the study group was treated with laparoscopic cryptorchidism descent surgery.Compare the treatment efficacy,general surgical conditions,degree of pain,complications,and follow-up between the two groups of children.Results The total effective rate of the study group was not significantly different from the control group(P>0.05).The levels of bleeding volume,hospital stay,and incision length indicators in the study group were lower than those in the control group,with significant differences(P<0.05).The pain scores of the study group were lower than those of the control group at 6 hours,12 hours,and 1 day after surgery,with significant differences(P<0.05).The incidence of complications in the study group was 5.00%(1/20),lower than that in the control group,but the difference was not significant(P>0.05).The comparison of follow-up results showed that there was no significant difference in testicular atrophy and retraction between the two groups of children(P>0.05).Conclusion The clinical effect of laparoscopic cryptorchidism descending fixation surgery for the treatment of high cryptorchidism in children is satisfactory,which can control trauma,shorten recovery time,reduce the occurrence of complications and pain level,and also ensure that children can achieve ideal long-term efficacy.
