两种置管方式在食管癌患者围术期应用的有效性与安全性研究

摘　　要：目的探讨改良式中长导管(中长导管)与经外周静脉穿刺植入中心静脉导管(PICC)在食管癌患者围术期应用的有效性与安全性。方法回顾性纳入我院2018年1月—2021年12月行食管癌根治手术的337例患者的临床资料,其中术前经静脉置入PICC患者254例,中长导管患者83例,经过1∶1倾向性评分匹配后,2组各纳入63例患者,比较2组患者的导管相关临床资料。结果经过倾向性评分匹配后,中长导管组平均置管时间及导管相关费用低于PICC组(P<0.05),但2组患者在置管成功率、导管内血栓形成率、非计划拔管、静脉炎、导管相关性感染率、导管堵塞率、平均输液时间方面的差异无统计学意义(P>0.05)。结论在食管癌患者围术期中,相比PICC,改良式中长导管置管时间更短,且其有效性与安全性并不亚于PICC,且能够节约医疗资源,降低患者医疗成本,值得在临床推广应用。Objective To explore the efficacy and safety of modified medium length catheter versus peripherally inserted central catheter(PICC)in the perioperative period of patients with esophageal cancer.Methods Clinical data of 337 patients who underwent esophagectomy in our hospital from January 2018 to December 2021 were retrospectively included,including 254 patients with preoperative PICC implantation and 83 patients with medium length catheter.After 1∶1 propensity score matching,63 patients in each group were included,and catheter-related clinical data of the two groups were compared.Results After propensity score matching,the average catheterization time and catheter related expenses in the medium length catheter group were lower than that in the PICC group(P<0.05),and there was no statistical significance in the success rate of catheterization,the rate of intracatheter thrombosis,the rate of unplanned extubation,phlebitis,the rate of catheter-related infection,the rate of catheter blockage,and the average infusion time between the two groups(P>0.05).Conclusion In the perioperative period of patients with esophageal cancer,compared with PICC,modified medium length catheters has shorter catheterization time,and the effectiveness and safety in modified medium length catheter are not inferior to PICC,and can save medical resources and reduce patients'medical costs,which is worthy of clinical application.
