摘 要:目的探讨心脏瓣膜置换患者术后行磁共振扫描的安全性。方法将SJM™Masters系列的人工机械心脏瓣膜和Carpentier-Edwards PERIMOUNT Magna Ease系列的人工生物心脏瓣膜放置于3.0T磁共振中进行体外测试,以评估人工心脏瓣膜与磁场的相互作用、热效应及伪影;对40例心脏瓣膜置换患者术后行磁共振检查并进行安全性观察。结果在3.0T磁共振系统中,本实验中测试的机械瓣膜在磁场中均未发生偏转和磁致扭矩,生物瓣膜发生轻度偏转和扭矩;在全身平均比吸收率(specific absorption rate,SAR)为3.4 W/kg的情况下扫描15 min后,所有人工心脏瓣膜均产生了<1℃的差异升温;伪影范围为轻中度;40例心脏瓣膜置换术后的患者于检查结束后未主诉不适症状。结论心脏瓣膜置换患者术后于3.0T磁共振检查中是安全可行的。Objective To explore the safety of magnetic resonance scanning in patients with heart valve replacement after surgery.Method The SJM™Masters Series of mechanical heart valves and the Carpentier-Edwards PERIMOUNT Magna Ease bioprosthetic heart valves were placed in a 3.0T magnetic resonance system for in vivtro study to assess the magnetic field interactions,heating and artifacts;Magnetic resonance examination and safety observation were performed on the patients after valve replacement.Results Mechanical valves were free of deflection and magnetic field-induced torque in the magnetic resonance imaging system,while bioprosthetic valves were slightly deflected and torqued.All prosthetic heart valves produced a differential heating of<1℃after 15 min of scanning at a whole body-averaged specific absorption rate(SAR)of 3.4 W/kg;The total range of the artifact was mild to moderate;40 patients did not complain of discomfort symptoms after the examination.Conclusion Postoperative 3.0T magnetic resonance imaging is safe and feasible in patients undergoing cardiac valve replacement.
