摘  要:目的探究慢性心力衰竭(CHF)患者血清环状RNA同源域相互作用激酶3(circHIPK3)和转录因子GATA结合蛋白4(GATA4)与心室重构及心功能的相关性。方法选取2021年6月至2023年6月河南科技大学第一附属医院收治的152例慢性心力衰竭(CHF)患者为观察对象,作为CHF组,选取同期健康体检者110例,作为对照组,参照纽约心脏病协会(NYHA)心功能分级标准将CHF患者分为Ⅰ~Ⅱ级(53例)、Ⅲ级(50例)、Ⅳ级(49例),采用实时荧光定量PCR法检测血清样本中circHIPK3表达水平,酶联免疫吸附法检测血清中GATA4蛋白水平,彩色多普勒超声心动图检测心室重构及心功能指标:左心室质量指数(LVMI)、左心室重构指数(LVRI)、左心室后壁厚度(LVPW)、左室舒张内径(LVEDD)、左心室短轴缩短率(LVFS)、心输出量(CO)、左心室射血分数(LVEF),多因素logistic回归分析CHF患者心功能分级的影响因素,Pearson法分析CHF患者血清circHIPK3、GATA4与LVMI、LVRI、LVPW、LVEDD、LVFS、CO、LVEF的相关性。结果与对照组比较,CHF患者血清中circHIPK3[(1.14±0.25)比(1.89±0.59)]、GATA4[(17.26±2.52)pg/ml比(26.94±4.24)pg/ml]水平显著升高,差异存在统计学意义(t=12.538、21.363;P<0.05),circHIPK3、GATA4水平随心功能分级的增加而升高。circHIPK3、GATA4、LVMI是心功能分级为Ⅲ~Ⅳ级的独立危险因素,CO、LVEF是保护因素(P<0.05)。CHF患者血清中circHIPK3、GATA4水平与LVMI、LVPW、LVEDD呈正相关,与LVRI、LVFS、CO、LVEF呈负相关(P<0.05);CHF患者血清中circHIPK3与GATA4呈正相关(P<0.05)。结论circHIPK3、GATA4在CHF患者血清中呈现高表达,与心室重构、心功能指标存在一定关联性,可能对该病的诊治、预后评估有重要参考价值。Objective To investigate the correlation between serum circular homeodomain interacting protein kinase 3(circHIPK3)and GATA binding protein 4(GATA4)and ventricular remodeling and cardiac function in patients with chronic heart failure(CHF).Methods From June 2021 to June 2023,152 patients with CHF admitted to the First Affiliated Hospital of Henan University of Science and Technology were collected as the CHF group;meantime,110 healthy examinees from the same our hospital were regarded as the control group.CHF patients were classified into gradesⅠ-Ⅱ(53 cases),Ⅲ(50 cases)andⅣ(49 cases)according to NYHA cardiac function grading standards.Real-time fluorescence quantitative PCR method was applied to detect the expression level of circHIPK3 in the serum samples,and enzyme linked immunosorbent assay(ELISA)was applied to detect the level of GATA4 protein in the serum.Color Doppler echocardiography was applied to detect ventricular remodeling and cardiac function indicators:left ventricular mass index(LVMI),left ventricular remodeling index(LVRI),left ventricular posterior wall thickness(LVPW),left ventricular end diastolic diameter(LVEDD),left ventricular fractional shortening(LVFS),cardiac output(CO)and left ventricular ejection fraction(LVEF).Multivariate logistic regression analysis was used to analyze the influencing factors of cardiac function grading in the CHF group.Pearson method was applied to analyze the correlation between circHIPK3 and GATA4 in the serum of CHF group and the LVMI,LVRI,LVPW,LVEDD,LVFS,CO and LVEF of the CHF group.Results Compared with the control group,the serum levels of circHIPK3[(1.14±0.25)vs.(1.89±0.59)]and GATA4[(17.26±2.52)pg/ml vs.(26.94±4.24)pg/ml]in CHF groups were obviously increased(t=12.538,21.363;P<0.05),and the levels of circHIPK3 and GATA4 increased with the increase of cardiac function grading.CircHIPK3,GATA4,and LVMI were the independent risk factors for cardiac function grading fromⅢtoⅣ,while CO and LVEF were the protective factors(P<0.05).The levels of circH
