接纳与承诺疗法对下肢截肢患者病耻感、伤残接受度及创伤后成长水平的影响

摘　　要：目的探讨接纳与承诺疗法对下肢截肢患者病耻感、伤残接受度及创伤后成长水平的影响。方法选取河南科技大学第一附属医院烧伤显微外科2020年10月至2022年10月期间收治的116例下肢截肢患者作为研究对象,按照干预方式分为护理组和疗法组。护理组58例给予常规护理干预,疗法组58例给予联合接纳与承诺疗法干预,对比两组患者病耻感,伤残接受度和创伤后成长水平。结果两组患者干预后6个月的经济歧视、社会排斥、社会隔离、内在羞耻感评分较干预前降低,且疗法组低于护理组(P<0.05)。两组患者干预后6个月价值范围的扩大、对身体形态的从属性、对伤残影响的包容、从对比价值到固有价值的转变评分较干预前升高,且疗法组高于护理组(P<0.05)。两组患者干预后6个月的人生感悟、个人力量、新的可能性、与他人关系评分较干预前升高,且疗法组高于护理组(P<0.05)。结论接纳与承诺疗法可提高下肢截肢患者伤残接受度,改善患者病耻感,提升其创伤后成长水平。Objective To explore the effects of acceptance and commitment therapy on stigma,disability acceptance and post-traumatic growth in patients with lower limb amputations.Methods A total of 116 patients with lower limb amputation in Department of Burn Microsurgery,the First Affiliated Hospital of Henan University of Science and Technology from October 2020 to October 2022 were selected as the study subjects and divided into nursing group and treatment group according to the intervention method.Fifty-eight patients in the nursing group were given routine nursing intervention,and 58 patients in the therapy group were given acceptance and commitment therapy intervention.The stigma,disability acceptance and post-traumatic growth between the two groups were compared.Results Six months after intervention,the scores of economic discrimination,social exclusion,social isolation and intrinsic shame in both groups were lower than those before intervention,and those patients in the treatment group were lower than those in the nursing group(P<0.05).Six months after intervention,the value range of the two groups was increased,and the change scores from relative value to intrinsic value were higher than before intervention,and the treatment group was higher than the nursing group(P<0.05).Six months after intervention,the scores of life perception,personal strength,new possibilities and relationship with others in both groups were higher than those before intervention,and those patients in the treatment group were higher than those in the nursing group(P<0.05).Conclusion Acceptance and commitment therapy can increase disability acceptance,improve stigma and enhance post-traumatic growth in patients with lower limb amputations.
