摘 要:目的探究德谷门冬双胰岛素对早期糖尿病肾病患者糖代谢水平、胰岛功能及氧化应激状态的影响。方法纳入2021年1月至2022年11月期间就诊于汝州市第一人民医院内分泌科的102例早期糖尿病肾病患者为研究对象,采用掷币法进行分组。对照组51例给予常规治疗,试验组51例在此基础上增加德谷门冬双胰岛素治疗。对比两组患者糖代谢指标、胰岛功能指标、氧化应激状态、不良反应。结果治疗后试验组患者空腹血糖(t=4.32,P<0.001)、餐后2 h血糖(t=5.30,P<0.001)、糖化血红蛋白(t=4.83,P<0.001)水平均低于对照组;试验组患者胰岛素抵抗指数高于对照组(t=4.33,P<0.001),胰岛β细胞功能指数(t=4.85,P<0.001)、空腹胰岛素水平(t=4.88,P<0.001)均低于对照组;试验组患者谷胱甘肽过氧化物酶(t=4.60,P<0.001)、超氧化物歧化酶水平(t=4.74,P<0.001)均高于对照组,丙二醛水平(t=4.96,P<0.001)低于对照组;两组患者胃肠道反应(χ^(2)=0.44,P=0.505)、乏力(χ^(2)<0.01,P=1.000)、头晕(χ^(2)=0.14,P=0.713)发生率差异无统计学意义。结论德谷门冬双胰岛素能够有效降低早期糖尿病肾病患者血糖水平,改善胰岛功能,降低氧化应激反应,且安全性高。Objective To investigate the effects of insulin Degludec/Aspart on glucose metabolism,islet function and oxidative stress in patients with early diabetic nephropathy.Methods A total of 102 patients with early diabetic nephropathy in Department of Endocrinology of Ruzhou First People’s Hospital from January 2021 to November 2022 were enrolled as research subjects and divided by coin toss method.Fifty-one patients in the control group received conventional treatment,and 51 patients in the study group received insulin Degludec/Aspart therapy.Glucose metabolism indexes,islet function indexes,oxidative stress state and adverse reactions were compared between the two groups.Results The levels of fasting blood glucose(t=4.32,P<0.001),2-h postprandial blood glucose(t=5.30,P<0.001)and glycosylated hemoglobin(t=4.83,P<0.001)in the study group were lower than those in the control group.The insulin resistance index of the study group was higher than that of the study group(t=4.33,P<0.001),the isletβcell function index(t=4.85,P<0.001)and fasting insulin level(t=4.88,P<0.001)were lower than those of the basic group.The levels of glutathione peroxidase(t=4.60,P<0.001)and superoxide dismutase(t=4.74,P<0.001)in the study group were higher than those in the basic group,and the level of malondialdehyde was lower than that in basic group(t=4.96,P<0.001).There was no significant difference between the two groups in incidence of gastrointestinal reaction(χ^(2)=0.44,P=0.505),fatigue(χ^(2)<0.01,P=1.000)or dizziness(χ^(2)=0.14,P=0.713).Conclusion Insulin Degludec/Aspart can effectively reduce blood glucose level,improve islet function and reduce oxidative stress in patients with early diabetic nephropathy,with high safety.
