趣味腹腔镜课程在腹腔镜基本技能培训中的应用

摘　　要：腹腔镜下熟练的缝合技巧是腹腔镜手术得以完成的保证,也是腹腔镜操作中较难掌握的技巧。对于腹腔镜初学者而言,如何高效地掌握腹腔镜下缝合打结技术是腹腔镜基本技能教学的一个重要课题。腹腔镜培训过程中发现趣味腹腔镜训练激发医学生学习兴趣及练习动力,在腹腔镜初学者缝合技能训练中起着重要作用。本研究把医学生分为两组,试验组是依次完成腹腔镜下剥葡萄、腹腔镜下剥桔子、腹腔镜下剪纸、腹腔镜下折爱心的训练及考核后,再进入腹腔镜下缝香蕉皮,对照组直接进行缝合训练,同等时间训练后,两组进行拾豆模块、钉转移模块、缝合打结模块的考核。结果显示,相比于对照组,试验组缝合考核合格率更高,钉转移模块完成时间更短。调查问卷显示学员们对趣味腹腔镜教学更感兴趣,最喜欢腹腔镜下剥葡萄和缝香蕉皮模块。趣味腹腔镜教学可提高外科医学生学习积极性及优化腔镜缝合打结技能培训的教学方法。Proficient suturing techniques under laparoscopy are essential for the successful completion of laparoscopic surgery and are often challenging to master.For laparoscopic beginners,efficiently acquiring laparoscopic suturing and knot-tying skills is a critical aspect of basic laparoscopic skills training.During laparoscopic training,it was found that the use of a fun laparoscopy course inspired medical students'interest in learning and provided motivation for practice,particularly in the training of laparoscopic suturing skills for beginners.This study divided medical students into two groups:the experimental group completed training and assessment in fun laparoscopy exercises(grape peeling,orange peeling,paper cutting,and folding a heart shape)before proceeding to laparoscopic suturing on a banana peel.The control group underwent direct suturing training.After an equal amount of training time,both groups were assessed using bean picking,nail transfer,and suturing and knot-tying modules.The results showed that the experimental group had a higher pass rate in suturing assessments and completed the nail transfer module in less time compared to the control group.Survey responses indicated that students were more interested in fun laparoscopy teaching and particularly enjoyed the grape peeling and banana peel suturing modules.Fun laparoscopy teaching can improve the motivation of surgical medical students and optimize the teaching method for laparoscopic suturing and knot-tying skills training.
