摘  要:目的 分析双膦酸盐类药物对区域骨代谢加速过程中巨噬细胞极化状态的影响。方法 选择40只雌性SD大鼠,随机分为空白对照组、唑来膦酸(ZA)组、白细胞介素-17(IL-17)组、拉喹莫德(laq)组,每组10只。ZA组、IL-17组、laq组腹腔注射唑来膦酸,空白对照组腹腔注射等量生理盐水。对四组大鼠行颌骨拔牙术, IL-17组大鼠给予抗大鼠IL-17中和抗体, laq组大鼠给予拉喹莫德。拔牙术后第8周处死大鼠,取完整骨固定,实施X线检查与Micro-CT扫描,分析其骨代谢情况。结果 空白对照组大鼠拔牙创愈合情况良好,无死骨形成,骨代谢尚可。ZA组、IL-17组和laq组大鼠拔牙创部位的骨体积分数(BV/TV)均明显低于空白对照组,差异具有统计学意义(P<0.05),说明ZA组、IL-17组和laq组骨代谢较快。空白对照组中的大鼠颌骨黏膜愈合良好,骨密度正常,骨代谢稳定。唑来膦酸有助于巨噬细胞向M1型极化,阻断或抑制IL-17活性可抑制巨噬细胞向M1型细胞极化。结论 双膦酸盐类药物会对区域骨代谢加速过程中巨噬细胞极化状态造成影响,临床应用时需要加以重视。Objective To analyze the effects of bisphosphonates on macrophage polarization state during the acceleration of regional bone metabolism.Methods 40 female SD rats were randomly divided into blank control group,zoledronic acid(ZA)group,interleukin-17(IL-17)group and laquinimod(laq)group,with 10 rats in each group.ZA group,IL-17 group and laq group were intraperitoneal injected with zoledronic acid,and blank control group was intraperitoneal injected with the same amount of normal saline.The four groups of rats were subjected to jaw tooth extraction.The IL-17 group was given anti-IL-17 neutralizing antibody,and the laq group was given laquinimod.The rats were executed at the 8th week after tooth extraction,intact bone was taken for fixation,and X-ray examination with Micro-CT scanning was performed to analyze their bone metabolism.Results The rats in the blank control group had good healing of tooth extraction wounds,no dead bone formation and fair bone metabolism.The bone volume/total volume(BV/TV)at the site of tooth extraction wounds of rats in the ZA group,IL-17 group and laq group was significantly lower than that in the blank control group,and the difference was statistically significant(P<0.05),which indicated that the bone metabolism in ZA group,IL-17 group and laq group was faster.The rats in the blank control group had good healing of the jawbone mucosa,normal bone density and stable bone metabolism.Zoledronic acid contributed to macrophage polarization to M1 type,and blocking or inhibiting IL-17 activity can inhibit macrophage polarization to M1 type cells.Conclusion Bisphosphonates can affect the macrophages polarization state during the acceleration of regional bone metabolism,which should be paid attention to in clinical application.
